0208568323 2016-06-20 Production Wells No Violations On the day of my inspection I found that the bare areas running downhill from the wellhead had been graded out, seeded and covered in straw. Approximately 60% of the grass was growing. Mark Fluharty met me at the site and he said that he would re-seed the area. The other area that was wet, sludgy clay and soil had been graded out and the exposed plastic material had been removed. The material was road fabric rather than pit liner. The dike had been cleared of all contaminated soil and filled in with new gravel. Identification had not been posted at the wellhead yet. Mark will contact me to inspect once he has the ID posted and the vegetation is growing.
0484027667 2016-02-01 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped with a pumping unit. The engine was intact and the valves were open. The tank battery consisted of two 100 barrel tanks, one separator and an earthen dike. Identification was posted and I found no leaks or spills. The tank battery is shared with permit 5797.
0784909817 2013-05-14 Plug / Plug Back No Violations On the day of my inspection Davis Well Service ran tubing to 6088. Cemenco broke circulation and pumped a 300 plug in the 4.5 casing from 6088 to 5788. Good circulation was maintained throughout the job and there were no leaks or spills.
0851136184 2016-02-22 Production Wells Violations Noted This inspection was conducted in response to a complaint of alleged pollution and contamination. I was joined by SE Regional Supervisor Brent Bear and NE Regional Emergency Operations Manager Terry Whitehair. On the day of our inspection we found that the well was being produced with a natural gas powered pumping unit. Mark Fluharty, pumper for Northwood, was on location to start the pumping unit. There was no identification posted at the wellhead. There was bare soil and erosion ruts leading downhill from the wellhead to an approximate 20 x 20 bare area that was heavily tracked by deer. There was a larger disturbed area approximately 50 feet from the wellhead that was bare of vegetation and very wet. We found patches of a thick grey substance that resembled plugging clay and we found exposed pieces of pit liner. The tank battery consisted of two 100 barrel steel tanks and one horizontal separator. An earthen containment dike was in place around the production equipment. The tank battery is shared with permit 5797. Identification was posted at the tanks. There was an area of oil/brine impacted soil mixed with sawdust under the load valves on one of the 100 barrel tanks. There was also a pile of oily sludge lying on the outside of the dike wall. The issues needing addressed at this well to bring it into compliance include: posting identification at the wellhead, removing and properly disposing of the impacted soil inside of the dike, removing and disposing of the oily sludge pile outside the dike, bio-remediate the impacted soils at the wellhead and establish vegetative cover.

Call from Bill Arnolt @ 4:30 pm w/ Northwood Energy 3/12/09 . 3/13/09 inspection of oil spill at well site . 210 bl. Storage tank inside dike , the dike contained all but 0.5 bls. Oil ran into drain tile at well site and came out down hill approximately 100 covering a area of 40 by 70 . Oil and contaminated soil have been removed from this area oil spill clean up is on going at this time . Storage tank is 25 from well head . Storage tank is in violation of spacing . Tank will need to at a minimum of 50 from well head & diked to contain 110 % of storage tank . No identification at well or tank . Called Bill Arnolt this date and notified of violations .

1669089934 2013-05-17 Plug / Plug Back No Violations On the day of my inspection Cemenco hooked up to the 4.5 casing to circulate through perforations. The pressure increased rapidly when attempting to circulate. The pressure was not allowed to increase over 1200 psi. Cemenco shut down without pumping any cement. The casing will need to be perforated again.

Identification was posted at the wellhead. The bare area below the wellhead was not growing. I called Mark Fluharty, pumper for Northwood and left a message to contact me.
1857571445 2014-09-15 Plug / Plug Back No Violations On the day of my inspection I found that Appalachian Well Surveys set an electric bridge plug at 4100 and then perforated the 4.5 casing at 4090 with four shots. Formation Cementing established circulation and squeezed 170 sacks of class A cement behind the casing to cement off the Clinton sand.
