0141811237 2016-12-27 Production Wells No Violations On the day of my inspection I found that the dike had been shortened and the walls built up. There was one new 210 barrel tank set and the horizontal separator was still in place. All of the ice in the dike had melted. The dike was full of water. I did not find any crude oil floating on the water. The old tanks and associated materials still need to be removed from the site.
0784694742 2017-05-16 Production Wells No Violations On the day of my inspection I met with DOGRM Emergency Operations Supervisor Trisha Cartmell to look at the site. We found that the site had been graded and cut with drainage berms to divert water from the area. We found no remaining crude oil around the site. The dike was still partially filled with water that was populated with tadpoles. There was an open dike drain in place. Trisha recommended that they remove the drain, or shorten the dike to only contain the 210 barrel tank. I called the pumper and let him know that he could move forward with seeding the site and covering with straw. I also spoke about the recommendation on the dike drain.
0856620645 2016-12-15 Production Wells No Violations On the day of my inspection I found that the 210 barrel tank that combusted had been removed from the home owners garden and staged back near the well site. The good 210 barrel tank and the bottom of the combusted tank had also been removed from inside the dike and staged near the well site. The 50 barrel steel tank and the separator were still in the dike. There was less water and oil in the dike than I found on 12/12/16, but the volume was difficult to determine due to snow cover. The oil soaked peat, absorbent pads and booms were still in place on the outside of the rear wall of the dike. Everything was frozen and covered with snow. I called Dan Walker, production supervisor for David Hill Inc. to get an update on the clean-up.
1291210887 2016-12-12 Production Wells No Violations I was contacted by NGO at 15:00 on 12/9/16 to report a tank explosion at this well. The incident was also reported to the DOGRM Emergency Response Hotline. I contacted the duty Supervisor Terry Whitehead, who said that he was sending Josh Schiering to respond. I spoke with Josh and he said that NGO was operating the well on behalf of David Hill, and therefore initially responded to start the clean-up. On the day of my inspection I found that there was an open bottom 210 barrel steel tank located in near a home, approximately 300 feet from the tank battery. The bottom of the tank was still inside of the earthen dike. The dike was half full of crude oil and water. There was oil soaked peat spread along the inside of the dike walls. I did not find any crude oil on the access road, or along the flight path of the tank., or around the tank where it was laying in the yard. There was a 40 foot by 40 foot area of oil soaked peat down the rear side of the dike wall into the edge of a marshy area below the dike. There were oil absorbent pads and booms in place to prevent the oil from getting into the marsh. I did not find any crude oil in the marshy area. I called Dan Walker, production supervisor for David Hill, who claimed that he was meeting with a contractor at the site today at 14:00 to do a walk through, and the contractor would finish the clean-up on 12/13/16. Dan said that they estimated 10 to 12 barrels of fluid lost, the majority of which was crude oil. He said that they have not determined a cause yet. Dan also said that they intended to transport the material to Kimbles landfill. He said that he would call me when they are finished with the clean-up. The neighboring homeowner came over to talk while I was on location. He said that he heard the explosion and witnessed the tank flying through the air. He speculated that the tank was shot by a hunter. I did find a small hole in the side of the tank. The hole was rectangular, approximately 0.25 by 1 inch, and in shar
1481727379 2016-12-14 Administrative Inspection No Violations I was contacted by Dan Walker, production supervisor for David Hill Inc. to let me know that a crew from Energy Services Corporation was on site working on the clean-up. They could not get the roll-off containers in yesterday due to the weather. Dan reported that the crew removed the other 210 barrel tank and the 50 barrel tank. They brought in a water truck and vacuumed another half load of fluid out of the dike. Dan and I discussed a plan to consolidate all of the oil impacted soil inside of the dike until they could get the roll-off trucks into the location.
1482251104 2016-12-20 Production Wells No Violations On the day of my inspection I found that there was a newly constructed, gravel lined earthen dike at the east end of the original dike. The original dike appeared to be free of crude oil, but it was difficult to determine due to the fact that the bottom of the dike was full of ice. I did not find any signs of crude oil in the ice. There was still a 10 foot diameter area of oily peat or saw dust on the upper portion of the outer dike wall. The lower portion of the wall and the perimeter of the marshy area showed signs of being scraped with the bucket of the back hoe. The absorbent pads and booms were frozen in place, and the general area was covered in ice. I did not find any signs of crude oil in the ice. There were still multiple small piles of yellow paraffin scattered throughout the area. I called Dan Walker, production supervisor for David Hill to report what I found. He agreed to meet me at the location to walk through the work that has already been done. I met with Dan Walker, Rick Reed of ESC, and Chase pumper for NGO. Dan, Chase and I walked through the marshy area and I pointed out the paraffin and oily peat that still needs to be addressed. Chase said that he would be out first thing in the morning to shovel the material up and remove it. Dan Walker did not agree with the position of the new dike. There is a natural spring at the location that was already draining into the new dike. Dan, Rick, Chase and I discussed a plan to use the existing dike and divert the spring water into the cattails. They brought in a new 210 barrel steel tank to be used. The old 210 and 50 barrel tanks will be removed. The existing separator will remain in place.
1482345395 2016-12-21 Production Wells No Violations On the day of my inspection I met with Rick Reed of Energy Services Corporation and Chase, the pumper for NGO. Chase used a shovel to remove the paraffin that was scattered around the marshy area below the dike. He also shoveled up the remaining oily peat on the rear outer dike wall. He removed most of the absorbent pads, but the booms were frozen in place. Rick eliminated the newly constructed dike and moved the new gravel to the existing dike for tank placement. He cut a channel to divert ground water and runoff around the dike and into the low lying marshy area. They set a new 210 barrel steel tank in the existing dike and connected the lines. The dike was still partially filled with ice and water. I did not find any signs of crude oil in the water. There was an existing dike drain in place. The dike will need to be drained once the ice melts. A truck showed up to remove the combusted tank. The other 210 barrel tank, 50 barrel tank, and various materials and debris were still staged near the well site.
2016580206 2017-03-30 Production Wells No Violations I received a call from NGO to notify me that they had removed the damaged tank and debris from the well site. On the day of my inspection I found that there were still areas of the site that needed work. I met with the NGO pumper at the site to go over the areas that still need to be addressed. There was a metallic sheen on the water that was pooled in a marshy area below the dike. When we walked into the area to inspect the sheen I found that it was not crude oil. The sheen was a result of iron leaching from the disturbed soil. There was also a floating scum on the water that I suspected to be oil, but it was algae. There is still one large area above the dike that needs to be graded out to the surrounding contour and seeded for vegetative growth. There were also ruts and ditches that need to be graded out and seeded where the old tank was dragged out. Finally, the dike is still holding a lot of water. Chase said that he would try to lower the dike drain and install a second drain when he was out with the excavating equipment to address the rest of the site.
