1390315042 2014-04-14 Production Wells No Violations FOLLOW UP TO CA # 1394728126: On the day of my inspection I found that identification had been posted at the well head. This violation is resolved.
1394728126 2014-03-13 Production Wells Violations Noted On the day of my inspection I found that the well was equipped with 4.5 casing that was topped with a fitting that connected to 1 steel line. The 1 steel line ran to a regulator then to a steel drip tank. I found 1 yellow plastic line running from the steel drip to the ground. The line was buried but it entered the ground in the direction of Paddy Fork Acres Farm, which consisted of a barn complex and a brick building with the windows busted out. There was no tank battery. I did find a horizontal poly tank next to the well head. I found no leaks or spills. There was no identification posted for this well. OAC 1501:9-9-05(A)10 states that all producing leases shall be legibly identified in a conspicuous place on or near the well-head or the storage tank(s) as to owner, lease name, well number, permit number where available, county, and an emergency telephone number.
