0063993591 2015-10-21 Production Wells No Violations On the day of my inspection I found that all of the brine water had been vacuumed up and the contaminated soil removed. I spoke with Bill Magrill from Petrox. He claimed that he accidentally broke the line to the 50 barrel tank when he was removing it, allowing some salt water to escape. This violation is resolved.

0303930243 2015-10-15 Production Wells No Violations On the day of my inspection I found that the access road to the wellhead had been opened up and cleared of brush. The wellhead was equipped as a swab well. There was a new ball valve on the swab head. The steel lines to the separator and from the separator to the 210 barrel tank were connected. The vegetation had been cleared from the tank battery and identification had been posted. I called Scott Dutton of Petrox. He claimed that they swabbed the well and it made 3 feet of fluid and some gas. He also claimed that they just shipped 80 barrels of oil from the 210 barrel tank. I requested that he provide documentation of the oil shipment in order to prove that this well is capable of production.

1275406549 2015-10-20 Production Wells Violations Noted On the day of my inspection I was driving through the area and noticed that the brine tank had been removed from the tank battery. I stopped to investigate and found a pool of standing water inside of the dike, where the brine tank had been set. The water was cloudy and it had an orange hue. I tested a sample of the water with a low range Quantab and the chloride level was off the scale. I tested the sample with a high range Quantab and the chloride level was above 6627 ppm. I called Petrox and spoke with Scott Dutton. He said that he would get Bill Magrill, his field man for the area, out to take care of it. I waited for an hour and no one showed. I called Bill Magrill and got no answer. On my way out of the area I ran into another Petrox employee who claimed that Bill was in the area, he was aware of the problem and he was planning to make it his next stop.
1312362705 2014-09-12 Plug / Plug Back No Violations On the day of my inspection I found that the well was still sitting idle. The tank battery was still overgrown with vegetation and there was no identification posted. The wellhead was still equipped with a Concoil lift system. I found the remnants of an electrical box for a pumping unit near the wellhead. The wellhead is located in a very desolate wilderness area and there were no signs that it had been serviced or operated in quite some time. This well currently has no production reported since 2009.
1376663342 2013-08-16 Production Wells No Violations FOLLOW UP TO VIOLATION # -1059243939: There were no signs of leaking or contamination at the well head or tank battery. Proper identification was still not posted. I will issue new violations for ID and non-production. This violation is resolved however, as there was no contamination present at the time of my inspection.

The tank battery was still overgrown with vegetation and there was no identification posted. I contacted Petrox. And spoke with Scott Dutton to remind him that this well has an outstanding violation to plug. He claimed that they would get a rig scheduled to get it done.
1447773974 2015-11-17 Administrative Inspection No Violations Scott Dutton of Petrox emailed me a copy of the haul ticket from Devco Oil showing 7 ft. 3 in of oil hauled from the MWCD 30 on 10/26/15. The well has been serviced and made capable of production. The well site and tank battery have been cleaned up and identified. This violation is resolved.
1751972073 2013-08-16 Production Wells Violations Noted On the day of my inspection I found that the well head was equipped with a Concoil lift system. The tank battery consisted of one 210 barrel stock tank and one 50 barrel brine tank. I found no leaks or spills at the well head or tank battery. The tank was heavily overgrown with vegetation, there were no physical signs (tire tracks or foot path) that the well has been serviced or produced recently. ODNR records show that there has been (0) zero production reported for this well since 2009. Finally, there was no identification posted at the well head or tank battery. Make well capable of commercial production within 30 days; or plugging is required within 90 days. Failure to comply may result in forfeiture of bond.
