0437537981 2014-10-31 Plug / Plug Back Violations Noted I found that Lake Region Oil had dammed up an intermittent stream in order to pull fresh water for the plug job. I noticed a sheen on the water where it was pooled up stream from the dam. I took a sample of the water and tested it for chlorides. The pH was at 6.0 and I found no salt in the water. I did get an oily film on my testing equipment. I found no point of release around the well site. The oil was believed to have come from the water truck. I found no oil down stream from the dam. I instructed Lake Region and Bakerwell to use absorbent booms and pads to collect as much oil off of the surface as they could, then push the oil to one corner and skim it off the surface with the vacuum truck. Once the pool is cleaned to my satisfaction they are to install two siphon dams down stream and release the water. They must remove the original dam and they are not permitted to alter the course of any stream in the future.

0597536787 2014-10-29 Plug / Plug Back No Violations On the day of my inspection I found that Bakerwell had 1.5 tubing ran to 6388. Petroset broke circulation and pumped a 30 sack cement bottom plug through the Rose Run. Good circulation was maintained throughout the job.
0783064907 2014-11-03 Plug / Plug Back Violations Noted On the day of my inspection I found that the pit liner had fallen in, allowing pit water to come into contact with the surface soil. I notified Bakerwell that they were not to proceed with cementing the casing string until the pit was drained and re-lined. The pit was drained and closed. They used a different pit for the cement job.
1328909204 2014-11-07 Completion Testing No Violations This wells longstring was cemneted and the latch down plug did not hold pressure at the time of the cement job. The well was shut in and the cement was allowed to set up. Pressured up on the 4-1/2 casing to 910 psig (4330 x .2 = 866 psig) and held for 15 minutes. The pressure dropped to 890 psig. Due to a small surface leak at the 4 x2 swedge. 20 psi is less than the allowed 45 psi so the mechanical integrity test passed.
1408493572 2014-11-03 Plug / Plug Back No Violations On the day of my inspection I found no visible signs of residual crude oil in the intermittent stream. A series of two siphon dams were used in conjunction with absorbent pads, booms and a vacuum truck to remove the oil. I gave authorization to remove the siphon dams and return the stream to its original course.
1415111060 2014-11-03 Casing String Witnessed No Violations On the day of my inspection Bakerwell ran used, tested 4.5 casing equipped with 5 centralizers to a depth of 4225. Petroset broke circulation and cemented the casing with 150 sacks of 50/50 Poz cement. Good circulation was maintained throughout the job. The pressure test failed upon landing the plug. They will try the pressure test again after the cement is set up.
1857584010 2014-10-31 Plug / Plug Back No Violations On the day of my inspection I found that Bakerwell had ripped the 4.5 casing at 5108 and pulled it from the well. They had 1.5 tubing set at 5077 for a rip/isolation plug above the Point Pleasant. Petroset attempted to break circulation and the tubing immediately pressured up to 1900 psi. They pulled three joints and Petroset was able to clear the tubing. Circulation was established and Petroset pumped a 200 cement plug from 4977 to 4777. Bakerwell pulled the tubing from the well.
2139361011 2015-06-15 Preliminary Restoration No Violations On the day of my inspection I found that the pits had been closed and the well site had been graded out to the approximate original contour. The vegetation was growing well and I found no signs of erosion. I found no drilling equipment or debris. This site passes restoration for the plug back.
