0888267564 2013-11-06 Production Wells No Violations On the day of my inspection Davis Well Service equipped the 2.875 casing with a 4.5 collar that was serrated on the bottom. They ran in to the obstruction and connected the power swivel. Formation Cementing connected to the swivel and pumped a mixture of bentonite gel and cotton seed hulls. Circulation was established and Davis Well Service was able to start drilling through the obstruction.

1384972031 2013-11-20 Plug / Plug Back No Violations Northwood Energy was authorized to plug the well from 2400 after making every reasonable effort to get back down to the top of the 2.875 casing. On the day of my inspection Davis Well Service had 2.875 casing set at 2400. Formation Cementing hooked up and began pumping when the pressure immediately increased to 700 psi. They shut down and Davis pulled four joints to attempt to clear the obstruction. Formation pumped on it again and the pressure increased to 1000 psi. They shut down and Davis pulled all of the 2.875 casing.
1385144010 2013-11-22 Plug / Plug Back No Violations On the day of my inspection Davis Well Service ran in with wire line tools to tag the plug. They got down to 2300 and determined that the plug had gone out into the formation. Davis ran 1.5 tubing to 2400. Formation Cementing pumped to ensure the tubing was clear. Then they pumped gel and lost circulation material, followed by 100 sacks of class A cement. Circulation was never established. Davis Well Service is required to tag the plug again before proceeding with any further plugging actions.
1385389535 2013-11-25 Plug / Plug Back No Violations On the day of my inspection Davis Well Service tagged the first plug at 1852 with wire line tools. Northwood was cleared to proceed with plugging. Davis ran 1.5 tubing to 1000. Formation made a reasonable attempt to establish circulation with 500 lbs. of gel and 100 lbs. of cotton seed hulls. Circulation was never achieved. Formation pumped a 200 cement plug from 1000 to 800 to cover the Berea. Davis Well Service pulled the tubing and set a brush and stone bridge at 500.
1385485487 2013-11-26 Plug / Plug Back No Violations On the day of my inspection Davis Well Service tagged the brush and stone bridge at 485. They made a reasonable effort to pull the 8.625 casing, but it would not move. Davis then ripped the casing at 340 and pulled it from the well.
1385958440 2013-11-27 Plug / Plug Back No Violations On the day of my inspection Davis Well Service ran 1.5 tubing to 450. Formation pumped class A cement till the well was filled to surface.
1386168610 2013-12-04 Plug / Plug Back No Violations I was contacted by Mark Fluharty to report that the cement fell approximately 10 to 15 feet. They topped the casing off with 4 bags of sack cement, cut the casing off 36 below grade and capped it with a steel plate.
1390160377 2013-11-21 Plug / Plug Back No Violations On the day of my inspection Davis Well Service had 1.5 tubing ran to 2000. Formation pumped 10 barrels to ensure the tubing wasnt plugged. Davis ran the tubing to 2400. Formation pumped 97 sacks of class A cement. Davis Well Service pulled the 1.5 tubing from the well. They will be back in the morning to tag the plug.
1431432697 2015-05-11 Final Restoration No Violations On the day of my inspection I met with Mark Fluharty, pumper for Northwood, to go over the restoration. When this well was plugged the site was extremely wet and muddy from natural springs running out of the hillside. I found that the pit was closed, and the production equipment was removed. The site was graded out to the approximate contour of the surrounding landscape. Mark left a drainage ditch through the middle of the site to help with the water. The site was approximately 70% growing with rye grass. The lease road had also been graded out and planted in rye. I requested that Mark do some more grading around the drainage ditch and re-seed.
1440515354 2015-08-25 Final Restoration No Violations On the day of my inspection I found that the drainage ditch had been graded out to blend more closely with the surrounding landscape, while still allowing for adequate drainage. The rest of the site and lease road were growing in well established grasses and broadleaf vegetation. Mark Fluharty was on site to seed the freshly graded area and cover it with straw.
1442095689 2013-11-05 Production Wells No Violations I received notification from Northwood Energy that they had a hole in the 2.875 casing at approximately 1500. They attempted to back the casing off below the hole in order to replace it and put the well back into production. Davis Well Service backed the casing off at approximately 2500 and pulled it. When they were running back in with 2.875 casing they found that the hole had caved in on them and they could not get the casing any deeper than 1600. They attempted to wash it in with a mud pump and power swivel with no success. They could not achieve circulation with their equipment.
1934437796 2013-11-12 Production Wells No Violations I received notification from Bill Arnholt of Northwood Energy that they have made approximately 500 of hole. They are still experiencing circulation problems. He wanted to plug the well where they are. I told him that they need to continue drilling out the cave in in an attempt to get back on the 2.875 casing.
