0130029401 | 2019-01-14 | Production Wells | No Violations | On the day of my inspection I found that RB Drillings rig was still rigged up on the well and the trailer of tubing was still staged at the site. There was no one working at the well. I called Mark Fluharty with Northwood to ask what the plan was for this week. He said that they had equipment to bring up from down south, and he expected that they would be in to work on retrieving the plunger sometime this week, weather providing. |
0227659814 | 2019-05-14 | Production Wells | No Violations | On the day of my inspection I found that the tubing and casing valves were shut in at the wellhead. The only gauge mounted at the wellhead was on the surface casing, and it was back down to 50 pounds. The sight glass on the Roots meter was full of condensation, but I could make out the last two numbers. I believe the reading at 12:30 on this day was at 32445. There was no gauge on the sales line to show lien pressure and I have not yet received a copy of Mark Fluhartys formula to calculate gas sales. I called Mark and told him that the well appeared to be shut in again. He said that he has been keeping the valve on the surface casing only slightly cracked open to keep the pressure at 50 pounds. He also said that theyve been out here three times and found the valve shut completely and he speculated that it was the landowner checking the pressure then shutting the valve, not knowing that it was to be left slightly cracked open. Mark said that he plans to be out here on Thursday and he would contact me so we can meet up. |
0410979585 | 2018-08-06 | Production Wells | No Violations | On the day of my inspection I found that the 2-7/8 inch diameter casing valve was open. The line from the plunger catcher was connected to steel flow line. I noticed on this inspection that the rubber gas line from the surface casing was disconnected and capped with a plug. There were no leaks around the wellhead. There were no signs that a service rig had been present at the wellhead. The 100 barrel steel tank was still empty, and there was not a gas production chart at the separator. The condensation line on the tank was clearly visible and showed less than 6 inches of fluid in the bottom of the tank. |
0504946838 | 2011-12-01 | Production Wells | No Violations | Well in compliancew and capable of production. |
0588840212 | 2019-06-07 | Production Wells | No Violations | I met Neal Caldwell at the well site to conduct a follow up inspection. I found that the tubing valve was open, but there was no gauge to determine pressure. The casing valves were closed and the gauge read zero. The flow line valve to the separator was closed. The Roots meter showed 32450, up from 32445 on 5-16-19, which equates to 1.33 MCF in three weeks. Neal said that he was temporarily using gas off the well for his stove and water heater, but it only lasted for about 7 or 8 days. I spoke to Mark Fluharty two days ago and he said that he has not had time to swab the well. |
1157545095 | 2019-02-25 | Production Wells | No Violations | On the day of my inspection I found that there has been no changes at the well site. Mark Fluhartys back-hoe was still parked at the site, and Northwoods mud pump was still there. I called Mark to ask when he was moving in to swab. He said that the was just finishing up on the well he was currently swabbing, and he hoped to be moved in by the end of this week. I asked Mark to call me when he was planning to start swabbing. |

1388123374 | 2019-04-23 | Production Wells | No Violations | On the day of my inspection I found that the reading on the Roots meter was up to 32427. The valve to the gauge on the casing gas line was closed at the wellhead. I checked the gauge and it showed 200 pounds. There were no signs that any other work had been done at the site. The tank was still partially full of fluid and Mark Fluhartys swab rig was still rigged up on the well. I have not had any recent communication from Northwood Energy on the status of this well. |
1399892159 | 2019-04-29 | Production Wells | No Violations | I was copied on an email this morning from landowner Neal Caldwell to Bill Arnholt with Northwood. Neal requested that Northwood remove their equipment from the well site because he was getting ready to have cattle put out to pasture on the property. When I arrived at the site I found the Mark Fluharty was there with his pumper. Mark removed the mud pump from the property and he brought his back-hoe down to the tanks. They also had a mini-hoe there to dig up the line that was leaking. Mark was attempting to run plastic in the steel line that we found leaking last week. Bill Arnholt called me earlier in the day to notify me that he had received the email, and he was instructing Mark to put a fence up around his swab rig. Bill said that he wanted to leave the rig there because once they get the line repaired he is going to have Mark swab the well again. Northwood maintains their position that the well is capable of production. |
1527687740 | 2018-05-30 | Production Wells | No Violations | On the day of my inspection I found that the well was equipped with a casing plunger, and the gate valve on the casing was open. The line from the plunger catcher to the steel flow line was connected and the valve was open. There were no gauges at the wellhead to monitor pressure. There were no gas or fluid leaks at the wellhead. The 100 barrel steel tank was almost empty. I climbed on top to check the fluid level and found that there was approximately 12 inches of fluid in the bottom, but I could not determine the composition of the fluid with regards to crude oil or production water. The lines were all connected at the horizontal separator, but there were no gauges to determine if there was any gas pressure. There was no production chart in the separator. There was a house gas meter and drip bottle at the separator; the valve was open and the gauge showed 15 pounds, but the dials were not turning on the house gas meter. There was not a containment dike around the production equipment. There were no signs of leaking and the identification sign was in place. |
1541167716 | 2018-11-02 | Production Wells | No Violations | On the day of my inspection I found that there were no changes at the wellhead or production equipment. The main valve was still open at the wellhead and the rubber gas line was still disconnected. There were no signs of leaking around the wellhead, and there were no signs that a service rig had been rigged up on the well. The tank still had approximately 6 to 8 inches of fluid in the bottom. There was no chart present, and no gauges to show gas pressure. There were no leaks or spills around the tank or separator. I called Bill Arnholt with Northwood Energy to ask about the well status. He did not answer so I left a voice message. I also left a voice message for Neal Caldwell to let him know that I would be out today to conduct an inspection on the well. Neal returned my call after I had left the site and asked if I could return to go back out to the well with him. Neal brought a gauge to check the pressure at the wellhead. When he opened the valve off the plunger catcher there was no audible indication of gas and I did not detect the odor of gas. Neal equipped his gauge and it showed zero (0) pounds of pressure. He also brought a string and weight to gauge the fluid depth in the tank. We found that the bottom of the tank had a layer of thick oily sludge that was right up to the load valve, approximately 6 inches off bottom. The sludge was so thick that the weight sat on top rather than sinking in. There was no chart at the separator. There have been no communications from Northwood Energy in regards to retrieving the lost/stuck plunger. This well is idle and not capable of production or domestic gas in its current state. There is no gas pressure on the well and there has been no fluid production throughout 2018. |
1544628251 | 2018-12-12 | Production Wells | No Violations | On the day of my inspection I found that there were no changes at the well site or production equipment. There were no vehicle tracks or indications that Northwood had a service rig in to retrieve the stuck plunger. The tubing valve was still open at the wellhead, and the casing line was still disconnected. The valves were still open at the separator and house gas meters. None of the dials were turning on the meters. The 100 barrel steel tank was still empty. There was no fluid in the tank., and the oily sludge in the bottom was still below the load valve, less than 6 inches. I spoke with the landowner Neal Caldwell and he said that they have not observed any oil and gas related activity on the property. He has not seen a pumper, or a rig , or a water truck. He also confirmed that they are not receiving house gas from the well. Neal said that he went out to check the tank in mid November and he found that there were no changes in fluid content. |
1545662425 | 2018-12-24 | Production Wells | No Violations | Neal Caldwell contacted me on 12/22/18 to notify me that there was a service rig moving in on the well. I had spoken to Derrick Buxton of RB Drilling earlier in the day and he mentioned that he was going to moving in for Northwood. On the day of my inspection I found that they were rigged up on the well and had started to run the tubing. No one was working at the site. I called Mark Fluharty to ask for an update. He said that they got rigged up and started running in the hole with fishing tool on Friday. He said that Buxton will not be back to work until Thursday 12/27/18. The plan is to fish the rabbit out and then swab the well if they are successful at retrieving the tool. Mark said that they would shut it in and let it set over the next weekend and then assess the wells potential for productivity after the first of the year. He said that they would be swabbing into the existing 100 barrel steel tank when they get to that stage. I asked Mark to keep me updated on their progress. |
1548340809 | 2019-01-23 | Production Wells | No Violations | On the day of my inspection I found that RB Drilling was preparing to run tubing in the well to circulate, in an attempt to free the plunger for retrieval. Derrick Buxton had concerns with trying to pump through the tubing string, so Northwood brought in Jeff Breamer to steam the tubing. Then Buxton ran the tubing in to a depth of 3680 feet. By the time he got the tubing ran it was late in the day, so he shut down and said that they would start circulating first thing in the morning. |
1548343346 | 2019-01-24 | Production Wells | No Violations | On the day of my inspection I found that RB Drilling was already rigged up and pumping water through the tubing. The well was not circulating. Derrick Buxton said that it was circulating good for the first half hour and then they lost circulation. They pumped for a while longer and then shut down to wait for more water. They got the water delivered and started pumping again. The fluid started circulating again once they began pumping. The mud pump was down for mechanical issues several times throughout the day. They ended up only circulating enough mud and sand out to fill half a five gallon bucket. Mark and Derrick decided to pull the tubing back out and make another run with the fishing tool. Derrick pulled the tubing string and shut down for the evening because there was not enough daylight left to make another run. |
1548435144 | 2019-01-25 | Production Wells | No Violations | On the day of my inspection I found that Derrick Buxton with RB Drilling ran the tubing string back in equipped with a fishing tool. They were successful at retrieving the lost plunger on the first run. I spoke to Mark Fluharty with Northwood and he said that the plan was to run the tubing back in the well to a depth of 3680 feet. He said that there was a plunger stand in the well at that depth. Mark will try to bring his swab rig back within the next couple weeks and swab the well to determine if it is still capable of production. Mark said that if he cant get any results with swabbing Northwood will apply for a permit to plug the well. |
1551278309 | 2019-02-27 | Production Wells | No Violations | On the day of my inspection I found that Mark Fluharty was moving in and rigging up to swab the well. Mark said that he had some work to do on his rig before he could start swabbing. He did not have an estimate as to when he would start, but he said that he would call me when he is finished with the rig repairs. Mark will be swabbing through the 1.5 inch diameter tubing. |
1551375367 | 2019-02-28 | Production Wells | No Violations | Mark Fluharty called me to give notice that he was ready to start swabbing the well. When I arrived at the location Mark was rigging his swab tools up to make the first run. He made 10 to 12 runs while I was at the well site to observe. The tubing head was plumbed to the flow line, so most of what was being swabbed out was going to the tank. He did briefly open a bleeder valve to blow into a bucket on each run, which resulted in short bursts of gas and thick, oily fluid. At no time during the day did the well kick off and start to blow gas on its own. Mark said that he would be back out tomorrow or next week, depending on the weather. |
1551467342 | 2019-03-01 | Production Wells | No Violations | On the day of my inspection I found that Mark Fluharty was finishing up with swabbing until next week. He said that he got 8 inches of fluid out yesterday and 2 inches today. Mark believed that he was loosing fluid back down the tubing when he was pulling the swab tools up, so he proposed installing a standing valve in the tubing to prevent the fluid from falling back down. Mark said that there was no significant gas show this morning when he started. He also said that there was no significant oil show in the swabbed fluid. It has mostly been water left over from circulating to free the stuck plunger. Mark said that he would keep me updated as to when he got the standing valve and when he would be back out to continue swabbing operations. |
1551891627 | 2019-03-06 | Production Wells | No Violations | On the day of my inspection I met Mark Fluharty at the well site to witness swabbing operations. The well was last swabbed on 3/1/19 and it was shut in until today. There was no gas pressure on the well when mark opened it up after being shut in for five days. The well was actually on vacuum when Mark opened it to send the standing valve down the tubing. Once the valve was in place Mark made about 6 or 7 runs with no significant gas production. He was still pulling water out from when the circulated to free the stuck plunger. Mark was having issues with his equipment due to the cold weather and he decided to shut down until he could get new swab cups. |
1553005656 | 2019-03-19 | Production Wells | No Violations | Neal Caldwell called me this morning to report that there was a water truck at the well site. When I arrived I found that Fluharty Pumping Service had a water truck at the well site to drain and close the pit. The pit was used to circulate to stuck plunger free, and it was kept open in the case that Northwood decided to plug the well. Mark closed the pit today because Mr. Caldwell wanted to put cattle back in the pasture. They left some of the pit fluid in the tank with the fluid that had already been swabbed from the well. Mr. Caldwell measured the fluid and we found that there was approximately 4 feet left in the tank. Mark said that he was planning to be back tomorrow to swab again. |
1553089381 | 2019-03-20 | Production Wells | No Violations | I was supposed to meet Mark Fluharty at the well site for swabbing this morning. When I arrived no one was working at the site. I called Mark and he said that he got held up with other priority issues. He said that he will call me when he is able to get back out to the site. |
1554990447 | 2019-04-11 | Production Wells | No Violations | On the day of my inspection I found that Northwood had installed a 1 inch diameter steel line on the surface casing, as they are now claiming the well is capable of gas production from the surface casing annulus. The line was equipped with three valves and a gauge. The valves were closed and the gauge read 0 psi. I called Mark Fluharty and he said that it was safe to open the valves to get a pressure reading. The gauge pressured up to 300 psi. Mark said that he planned to open the line tomorrow morning and start running the gas through the separator and meter. I tried to get an initial recording on the meter but the sight glass was full of condensation. Mark said that when he comes out tomorrow morning he will remove and clean the sight glass, and take a photo of the dials before he opens the valves. The tank was close to half full with fluid of an undetermined composition. They also has a new valve on the production casing, which was open. Mark admitted that the well was not making any gas or oil from the production casing at this time, and he has not had time to swab any more since our last session. |
1555078380 | 2019-04-12 | Production Wells | No Violations | On the day of my inspection I found that Mark Fluharty had been out to the well this morning to open the valves on the casing gas line. Mark opened it up around 08:00 and sent me a picture of the reading on the rotary meter showing 32410 cubic feet. I arrived at the site at 09:45 and the meter reading was at 32413. The gauge at the wellhead was down from 300 to 150 pounds. I could hear gas coursing through the line and the meter dials were turning. I asked Mark about the reading and he said that you need to use a formula to translate the reading into actual cubic feet of gas. I will check back on Monday morning and get a reading then. |
1555338276 | 2019-04-15 | Production Wells | No Violations | I stopped to check the gas pressure and meter reading after the weekend. On the day of m y inspection I found that the gauge on the casing gas line was down to 20 pounds. The reading on the meter was 32426. Neal Caldwell sent me photographs on Saturday morning April 13, 08:30 showing 30 pounds on the gauge and 32423 on the meter. Mark Fluharty sent me a photograph on Friday morning April 12, 08:46 showing 32410 on the meter, and I personally saw the gauge reading 300 pounds. Within 24 hours the casing pressure dropped from 300 to 30 pounds and the gas production was minimal. I will check back later this week to get another reading. |
1556116570 | 2019-04-24 | Production Wells | No Violations | On the day of my inspection I was accompanied to the well site by inspector John Fleming. We found that there was 200 pounds on the surface casing gauge. I called Mark Fluharty to ask if he would come out to meet us at the site and put a gauge on the tubing. Mark said that the well had been shut in since last Tuesday. When Mark first opened the valve on the tubing, the pressure went up to 75 pounds and then started to drop off. Mark went down to the separator to check for leaks and found that they had a leak in the flow line. The landowners Mr. and Mrs. Caldwell showed up and Mark went through the separator configuration with everyone. Mark put a gauge on the check valve and he said that he would provide the calculation that he uses to determine actual cubic feet of gas sales. John questioned the positioning of the Roots meter, and Mark said that he had it turned that way so the I could see the reading. Once he gets the line leak addressed we will start monitoring the gas production on a weekly basis. |
1556807233 | 2019-05-02 | Production Wells | No Violations | On the day of my inspection I found that a steel livestock fence had been put up around the swab rig and wellhead. There was no other equipment left at the wellsite besides the swab rig. The areas that Mark Fluharty had to dig up to repair the flow line were filled back in and graded flat. Marks backhoe was still parked by the separator. I spoke to him on the phone and he said that he should have the backhoe moved out by the end of the day, or tomorrow at the latest. The tubing valve was closed at the wellhead and the gauge had been removed. The surface casing valve was open and the gauge showed 30 pounds of gas pressure. The wheels on the Roots meter were not turning when I was at the site. The reading on the dial was at 32437. Mark said that he opened the casing valve on Monday, once they finished with running plastic in the flow line. I also spoke with Neal Caldwell and let him know that Northwood did have a fence up around the rig and wellhead. |
1556899285 | 2019-05-03 | Production Wells | No Violations | On the day of my inspection I found that Mark Fluhartys backhoe had been moved off the property. The only equipment left at the site was the swab rig, which was fenced to keep the cattle out. |
1557411946 | 2019-05-09 | Production Wells | No Violations | On the day of my inspection I found that there were cattle in the Caldwell property. The livestock fence around the swab rig was securely in place. I found no leaks or spills around the swab rig, wellhead or production equipment. The valve on the surface casing line was open to the gauge, which showed 160 pounds. The valve to the flow line was closed, and the tubing valve was closed. There was not a gauge on the tubing. The Roots meter reading was still at 32437. I called Mark Fluharty and he said that the well was supposed to be open to sell gas. He did not know why the valves were closed and he said that he would be out today to open it back up. Mark also said that he has not had a chance to swab any more. |

1823265419 | 2018-02-12 | Production Wells | No Violations | I received a notification on Friday 2/9/18 that there had been an idle well complaint call from the landowner Mr. Neal Caldwell. I contacted Mr. Caldwell to get some basic well information and agreed to meet at the site on Monday. On the day of my inspection I found that the well was equipped with 2.875-inch diameter casing that was set up for a casing plunger. The main valve on the casing was open. There was a rubber line connected from the top of the tool catcher to the steel flow line that entered the ground next to the wellhead. The valve on the rubber line was also open. The production equipment consisted of one 100- |

Mark was digging a pit and the RB crew was lining it. We discussed the plan of action. I was told that they were bringing Northwoods mud pump up here tomorrow and they would be circulating through the tubing on Monday, 1/21/19, which is s federal holiday. They hope to clear any sand or mud that may be preventing them from getting a firm grip on the plunger with their fishing tool. I will follow up on Tuesday. |
1938084201 | 2018-03-01 | Production Wells | No Violations | On the day of my inspection I found that there were no changes at the well site. I called Bill Arnholt with Northwood Energy. I notified Bill that there had been a landowner complaint on this well. I conveyed the landowners claims to Bill and I asked for his response. Bill confirmed that the Concoil plunger tool was stuck downhole and they were unsuccessful at retrieving it with Mark Fluhartys swab rig. Bill speculated that the 14 barrels of oil reported for 2016 annual production was likely from prior swabbing attempts. Bill also agreed that the well has not been in production since 2015. I asked him for a verbal commitment to either put the well in production or plug. I also suggested a timeline for bringing a rig in to retrieve the plunger. Bill would not commit to anything. He claimed that he would look into it the next time he had a service rig in the area, and if he had a string of 1.5 inch diameter tubing. |
1964777469 | 2019-01-22 | Production Wells | No Violations | On the day of my inspection I found that RB Drilling was not working at the site. The tubing was still on the trailer. I spoke to Mark Fluharty with Northwood, and he said that Derrick Buxton was out early this morning, but they shut down due to the weather. The temperature is in single digits and the equipment was frozen. I will return tomorrow. |
2036930359 | 2019-02-11 | Production Wells | No Violations | On the day of my inspection I found that the service rig and associated equipment had been moved out. The tubing string was in the well and the wellhead was back together. Mark Fluharty still had his track hoe and mud pump staged back at the wellhead. There were no changes in the fluid level in the tank and the well was not making any gas. Mark said that he planned to move his swab rig in to swab the well before Northwood decides whether they will plug it or try to produce it. |
2063439150 | 2019-01-03 | Production Wells | No Violations | On the day of my inspection I found that RB Drilling was still rigged up on the well. The tubing string was laying on the trailer. No one was working at the site. I spoke to Mark Fluharty with Northwood to ask what they were doing. He said that they ran tubing in last week to fish for the plunger. Mark said that they were able to run the tubing all the way to bottom, so they likely pushed the stuck plunger downhole. Mark also said that they were no successful fishing the tool out of the well. He told me that they were waiting on word from Bill Arnholt as to their next plan of action. They have not swabbed the well yet and no fluid or gas was made during the tubing run. |
