0008815392 2018-08-22 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was preparing to bail the oil out of the hole. Phil ran the pre-cast cement plug to the 7 inch casing seat at 1055 feet, and he had oil on his tools when he came out of the hole. Phil dug and lined a small pit, and he bailed approximately 50 barrels of oil out of the well.
0211582570 2018-08-28 Plug / Plug Back No Violations On the day of my inspection I found that the rig was moved out and the clay pile had been pushed up next to the open well bore. The clay top was only down about 5 feet and it was solid. Phil Moran called me on Monday to inform me that he had been out to wash clay in the well for the top plug over the weekend. He said that he filled it from the pre-cast at 1055 feet to surface and left it open, and he was coming back out Monday evening to top it off again. I estimated that there was 5 to 6 tons of clay left in the pile, which would work out to be 7 tons for the top plug. I witnessed Phil washing 2 tons on the bottom prior to ripping the production casing. He then claimed that he was out to wash another 7 tons on the bottom plug over the previous weekend. That portion was not witnessed. The clay that was washed on the top plug this past weekend was also not witnessed. I sent a text message to Brandon with Capstone Oil to let him know that they will need to complete the plugging affidavit, as most of the actual clay plugging was done on weekends and not witnessed by a state inspector. Phil showed up at the site while I was there. He was taking as much of the remaining clay as he could get loaded onto his trailer. He also had a steel plate to cut and cap the well.
0489978025 2018-08-15 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was rigged up and pulling the tubing from the well. The clay was delivered while I was at the site. Phil got the tubing out and washed 2 tons of clay down the 4.5 inch casing.

0616590029 2018-08-16 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was rigging up his ripping tools. No more clay had been washed down the well since yesterday. Phil shut down to haul the tubing to a pipe yard and to bring a new trailer in for the production casing.
0726048324 2018-08-24 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was rigging up to part the surface casing. He could not get the casing to pull free and he had very little stretch when he pulled in it. Phil got a good telegraph at 380 feet and tried to rip the casing. His knife kept coming off and he never got any show of water, even though he had a good telegraph. Phil came up to 350 feet and tried for a while longer with the same issues. Phil kept coming up the hole until he eventually got the surface casing parted at a depth of 303 feet. He pulled it from the well and

1371010163 2018-08-20 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was not working at the well site. The pipe trailer was gone and the clay pile was smaller than it was at my last visit. I spoke to Phil on the phone and he said that he was out on Saturday to work. He said that they washed another 7 tons of clay down the well for a total of 9 tons on the bottom and he pulled all of the 4.5 inch casing from the well. Phil said that he did not run the pre-cast cement plug yet, and he has not tried to pull on the surface casing yet.
1524369933 2018-08-13 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was moving in to plug the well. Phil had his rig and equipment moved in, but he was not rigged up on the well yet. The pumping unit was still on the well and everything was still connected. Phil showed up when I was leaving the site. He said that he was going to try to get the rods and tubing out tomorrow and he should be ready with clay on Wednesday.
1534540848 2018-08-17 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran got a telegraph and parted the 4.5 inch diameter casing at a depth of 3080 feet. Phil pulled up one joint and shut down for the day due to excessive rain.

All of the plugging materials and debris had been removed from the site, and the pit was closed. I met with Brandon Wallace of Capstone Oil to go over the plugging affidavit and the final restoration requirements.
