0420092963 2016-11-18 Final Restoration No Violations I received a hand-delivered, signed copy of the form 5 landowner waiver from Randy Moran to waive the establish vegetative cover requirement of the final restoration. On the day of my inspection I found that all of the oilfield related materials and debris had been removed from the site. The former tank battery site and well site had both been graded out. There was no formal access road to the well, and the ingress/egress path used to plug the well had been graded smooth and was covered in grass. The actual well site was still bare and the pile of gravel from the pumping unit was still in place, as per request of the landowner. I did not find any signs of erosion problems at the site. This site passes final restoration.
0906652555 2016-10-13 Final Restoration No Violations On the day of my inspection I found that all materials and debris had been removed from the site. The well site had been graded out flat, and the tank battery site had been graded back into the contour of the hillside. The landowner did come out while the well was being plugged to request that the well site be left flat, and to request that he could use his own seed to establish vegetation on the site. I gave Randy Moran a form 5 landowner waiver to pass on the to well owner, so they could get parts of the restoration waived.

1473172667 2016-09-06 Plug / Plug Back No Violations On the day of my inspection I found that Randy Moran was rigged up to plug the well. The pumping unit had been removed, and Randy was pulling rods. They planned to pull all the rods and tubing today, and have the clay delivered tomorrow. Randy claimed that he was getting the clay from Logan.
1473432548 2016-09-08 Plug / Plug Back No Violations On the day of my inspection Randy Moran washed 2.5 tons of clay down the 4.5 inch casing. When Randy was rigging up to rip the casing a neighboring landowner stopped at the site. Allen Donaker told us that he remembered the well having a hole in the casing, and the owner washing clay down the annulus around the 4.5 inch casing. Randy was able to get some stretch on the casing. He went in with ripping tools and started looking for a telegraph at 2500 feet. He ended up getting the casing parted at 2000 feet. Randy pulled 20 joints and shut down for the day.
1473776227 2016-09-12 Plug / Plug Back No Violations Randy Moran reported that he bailed approximately 3 barrels of oil out of the well over the weekend. Rand could not get the 8.625 inch casing to pull free, so he had to rip. Randy parted the casing at 370 feet. He pulled the 8.625 casing from the well. Randy washed 11.5 tons of clay down the well. The well filled to surface and was allowed to settle multiple times throughout the day. The clay stayed at the surface after being filled one final time, with approximately 0.5 tons of clay left on the ground. Randy shut down for the day and left the well open to settle over night.
1474032431 2016-09-15 Plug / Plug Back No Violations On the day of my inspection I found that the well was still open. The clay was still sitting firm at the surface. I called Randy Moran. He said that he was planning to let the well sit open over the weekend, and he would be out the following week to close it.
1474469169 2016-09-21 Plug / Plug Back No Violations On the day of my inspection I met with Randy Moran at the site to witness the well being closed. The casing was still filled to the surface with clay. Randy excavated three feet down around the casing. He cut the casing off, and capped it with a steel plate mounted to a section of 4.5 inch casing that was pushed down into the clay. The plate was inscribed with the permit number. Randy was also working to remove the remaining production equipment. The landowner came out to the site to discuss the restoration. The well site was built into a hillside. The landowner said that he wanted the area to remain flat, rather than be graded out to the slope of the surrounding landscape. There was also a large pile of gravel that the landowner asked to be left at the site. He also said that he would seed the site himself after Randy did the final grade. I went over the form 5 landowner waiver with both parties, and left a copy with Randy.
1480371989 2016-09-09 Plug / Plug Back No Violations On the day of my inspection Randy Moran washed another 6 tons of clay down the 4.5 inch casing for a total of 8.5 tons on the bottom plug. Randy pulled the remaining 4.5 inch casing from the well, and set a pre-cast cement tapered plug on the 7 inch casing seat at 850 feet. There was oil present in the fluid column, so Randy brought in a bailer to bail the oil out of the well prior to pulling the 8.625 inch casing.
1597676759 2016-09-07 Plug / Plug Back No Violations On the day of my inspection I found that Randy Moran was pulling the tubing from the well. The clay was delivered while I was at the site. There was 20.5 tons of clay delivered from Logan Clay. The water truck also delivered a load of fresh water while I was on site. Randy said that he was taking the tubing back to his yard and getting more supplies to start running clay. He said that he would not put any clay down the well until the next morning. I told Randy that he must call me if he comes back out today to start washing clay down the well.
1947125096 2016-09-13 Plug / Plug Back No Violations On the day of my inspection I found that the clay was still sitting firm at the surface. I called Randy to let him know the clay had not fallen, and he could finish cutting the casing and capping the well. I made sure that Randy was aware that he could not orange-peel the casing. It must be capped with a plate. He agreed and said that he would make up a plate. I asked Randy to notify me when he would be out to cap the well.
