0171665120 2016-08-30 Administrative Inspection No Violations I received a phone call from landowner David Wigginson, who left a voice message claiming that he found a leak in the flow line from the wellhead to the tank battery. In the message Mr. Wigginson claimed that there was a hole full of oil and it was bubbling gas. I tried to call Mr. Wigginson multiple times and I could not get an answer. I left him a message to contact me at his earliest convenience. I called Mike Ingalls and Carrie Moore with KMFAD. Mike said that they did have a leak in the flow line and he had been out to make the repair. He dug the soil out around the leak to access the line. Mike said that he had the wrong size line clamp and he was on his way back out with the correct clamp. He claimed that the well had been shut in since the leak was discovered, thereby disputing Mr. Wigginsons claim of bubbling gas. Mike also disputed the claim that the hole was full of oil. He said that there had been a small amount of oil from the initial discovery of the leak, and the hole was full of water from recent rain. Carrie Moore sent me photographs of the hole to show that it was not full of oil. Mr. Wigginson finally called me back. He admitted that the excavated hole had been covered with piece of metal and since he only lifted the corner of the metal, that the fluid in the hole might have been rain water with a skim of oil on top. I told him that I had spoken to Mike Ingalls, and he was allegedly on his way out to finish the repair and clean up any oil. I was on sick leave at the time of the call. I asked Mr. Wigginson if I needed to send someone out. He sad that there was no need and he did not want to file an official complaint. He also claimed that he had a problem with an area in his hayfield that had been excavated by KMFAD for flow line repairs. He said that the soil had sunken down 7 or 8 inches, causing problems with his machinery when he mows hay. I told Mr. Wigginson that I would be out to follow up on September 6.
0305475466 2019-04-29 Production Wells No Violations On the day of my inspection I met Mr. Wigginson at the well site. He said that Carrie had been out last week to start laying a new line, but she did not finish and hasnt been back. There was a ditch started at the wellhead, and there was a 2 inch diameter plastic line plumbed into the steel flow line at the wellhead. The line was not buried and it was not connected at the separator. There was oil laying on the water that had collected in the ditch. The oil in the grass from the original leak had not been cleaned up yet either. I tried to call Carrie Moore and she did not answer. I left a voice message.
0341165876 2019-05-09 Production Wells No Violations On the day of my inspection I found that Carrie Moore had the new flow line buried from the wellhead to the separator. The ditch line was rough, but Mr. Wigginson said that he was going to disk the entire area as soon as it dries out a little. There was still some oily peat left on the grass where the old line had leaked. Mr. Wigginson said that he was going to spread some fertilizer on it and disk it in with the rest of the soil. He said that he was satisfied with what was done, and he would call me if he had any more issues. This compliance notice is resolved.

1434295389 2019-05-02 Production Wells No Violations On the day of my inspection I found that Carrie Moore was at the site working on the new flow line. The line had been properly tied in at the wellhead, and the oil that I found in the ditch was cleaned up. She still needed another section of steel line to tie in with the separator. She also still needed to get the oil cleaned up that was in the grass where the old linen had leaked. Carrie claimed that she would have the line all tied in today and then she wanted to pressure test the whole line before burying it.

1993161577 2019-04-16 Production Wells Violations Noted I received a phone call from landowner David Wigginson to report a leak between the pumping unit and the production tank. I met Mr. Wigginson at the site and he showed me the area of concern. There was an area approximately 3 x 5 feet that was laying wet with oil and water. Natural gas was bubbling up through the fluid. KMFAD had a similar incident in the same spot in August of 2016. I tried to call Carrie Moore and she did not answer. The voice mail was full, so I couldnt leave a message. I sent her a text message and she finally responded at 05:47 on 4/17/19. There was also a gas leak from one of the fittings on the production equipment. Mr. Wigginson said that his pressures have been unusually high. He said that he has always been maintained around 20 pounds, but he has noticed pressures up to 100 pounds recently. The gauge on the separator showed 70 pounds. The well owner shall complete the following actions within 5 days of the receipt of this compliance notice: (1) Identify and address the cause of the system becoming over pressurized. (2) Tighten the fitting at the separator to prevent the escape of gas. (3) Remove the oil contaminated soil from the area where the flow line is leaking. (4) Replace or repair the section of flow line that is failing.
2099358367 2016-09-06 Production Wells No Violations On the day of my inspection I met landowner David Wigginson at the well site. The well was being produced with an electric powered pumping unit. The unit was in operation at the time of inspection. The tank battery consisted of one 100 barrel steel tank and one horizontal separator. There was no dike around the production equipment. Correct, legible identification was posted on the tank. There were no leaks or spills around the tank battery. There were no active leaks at the wellhead. There was a four foot area of oil stained soil around the wellhead that had been stirred up and covered with mulch. Mr. Wigginson said that the stuffing box had a history of leaking. The area where the flow line had been repaired was covered with soil. I did no observe any oil staining in the soil. Mr. Wigginson showed me the disturbed area in the hayfield that he wants restored. It was rough and covered in weeds rather that hayfield vegetation. I told Mr. Wigginson that I would contact KMFAD and talk to them about restoring the area in the hayfield, and I would have them clean up the oil stained soil around the wellhead. I called Mike Ingalls with KMFAD to tell him that they needed to either remove the oil stained soil around the wellhead, or treat it with lime and fertilizer. I also asked about the disturbed area in the hayfield. He said that they were ready to go in and complete the restoration, and he would call me when they were done with everything.
