I spoke with Southeast District Supervisor Mike Moreland about the request. We agreed to grant the extension only after contacting Carrie to get a more detailed account of the action plan for the well. I called Carrie and spoke with her about her plan. She claimed that they were in the process of reconnecting the electric box, and installing a high speed gear in order to pump the well. I asked if she could meet me at the well to demonstrate the actions being taken to put the well back into production. She agreed to meet me on 8/19/16. I will grant the extension if she can show that they are actively working to resolve the issues preventing the well from being produced. |

The electric was connected to the motor and in the box, but the breaker was in the off position. The flow lines were connected at the wellhead, but the all of the valves were closed. The main valve to the separator was also closed. There was a gauge on the separator that showed 2 pounds. The 100 barrel steel tank was empty. No production reports have been filed for 2015. I called Carrie Moore about the status of this well. She claimed that they had treated the well with solvent and it was capable of production. She said that she has been in legal negotiations with the landowner George Hartsock and that has prevented her from doing anything with well. She claimed that they have come to an agreement and she was scheduled to put the well into production on 2/15/17. She also said that she has contracted Jim Dunfee to pump the wells for her. |
0844534268 | 2016-04-19 | Production Wells | Violations Noted | This inspection was conducted in response to complaint #5306. On the day of my inspection I met with the landowner George Hartsock to inspect the well. I found that the well was equipped with an electric powered pumping unit. The surface casing head was buried and not accessible. The tubing valves were both closed. The electric was disconnected at the box. The belts were off of the motor, and the drive shaft and sheave had been removed from the gear box. I conducted a field inspection of this well on 4/30/15 and found the well to be in the same condition. The inspection documents that I contacted Carrie Moore about the well, and she claimed that parts were on order to repair the pumping unit. Mr. Hartsock claimed that the well has been sitting in its current condition for 3 years and he wants the well plugged. The tank battery consisted of one 100 barrel steel tank and one horizontal separator. There was no earthen dike around the production equipment. The tank battery is shared with permit 3445. The identification on the tank only shows information for permit 3481 and there was no identification posted at the wellhead. I found no leaks or spills around the wellhead. There was some staining on the ground under the motor valve on the separator. The well is incapable of production in its current condition. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. |

The tubing and casing valves were closed. The polished rod showed some signs of recent use. I called Carrie Moore to get an update on the status of the well. She said that it is currently capable of pumping, but there was quite a bit of paraffin downhole that was diminishing there fluid and gas production. She claimed that the well was pumping too fast and they were currently in the process of procuring a jack shaft to decrease the strokes and start to pump the paraffin out. I reminded her that the compliance notice was due to be resolved by 10/19/16. She said that they would get to work on it, and asked that I meet her out here on 10/18/16 to observe their progress. |

Mr. Hartsock called to report that his house gas was shut off, and he wanted to know what she was doing to the well. He was also wanted to know if she was allowed to put the well back into production rather than plug, and what she had to do to bring the well into compliance with regards to production. I told Mr. Hartsock that she had until October 19, 2016 to either plug the well or demonstrate that it is capable of viable production. I also told Mr. Hartsock that minimum production is defined as 100 mcf gas or 15 barrels of oil in one year. |

Mike had a container to catch fluid attached to a nipple off of the tubing. He claimed that he thought he could pump the paraffin out. The jug was empty when he shut the unit down. Mike claimed that the rods and pump were free, but there was too much paraffin downhole to make any fluid. He claimed that the well was making gas, but had no was of proving that gas was being produced in commercially viable quantities. I reminded Mike that they have had six months to either make the well capable of production, or have it plugged. I recommended that he take immediate actions, such as solvent treatment, or steaming the tubing and rods to remedy the paraffin. I also reminded him that the well has a complaint filed on it and a compliance notice that was due today. I also spoke with Carrie Moore of KMFAD to discuss the same options. I am not granting them an extension. |

One was mounted to the unit and the other was laying to the side. There were no drive belts to connect the motor to the gear box. There was a solvent drum next to the wellhead and the lines were connected. The tubing valves were closed and the casing valve was partially opened. I tried to call Carrie Moore and she would not answer her phone. This well was required to be plugged and abandoned by 10/19/16. Nothing has been done recently to put this well back into production, and there has been no application submitted for a permit to plug this well. |

There has been zero oil or gas production reported for this well since 2016. This well has an outstanding compliance notice to be plugged, with a remedy date of 10/19/2016.
