1514909562 2018-01-02 Production Wells No Violations A phone call was made to the Zanesville office on 12/29/17 to report that the house gas had been shut off from this well. On the day of my inspection I found that there were fresh vehicle tracks in the snow leading back to the well, which was equipped with an electric powered pumping unit. The motor, drive belts and electric were intact. The flow lines were connected at the wellhead and the polished rod showed signs of recent use. There was some fresh oil spray and peat moss on the snow around the wellhead from a stuffing box leak. The oil was minimal. The tubing and casing valves were open. There were gauges on both nipples that showed 5 pounds on the tubing and 0 pounds on the casing. The production equipment consisted of one 100 barrel steel tank, one 50 barrel steel tank and one vertical separator in a shallow earthen dike. Correct, legible identification was posted on the tank. I found a house gas meter and regulator plumbed to the separator. The lines were connected and the valves were open. The gauge on the house gas line showed 0 pounds. I tried to call Dean Moran, but I git no answer and there was no option to leave a voice message. The property at 33216 TR 127 was locked up with a steel gate.

1841499927 2012-01-31 Production Wells No Violations The well is equipped with a pumping unit, 1-100 barrel, 1-30 barrel storage tank and a gas separator. The well is capable of production.
2140686275 2016-11-14 Production Wells No Violations On the day of my inspection I found that the well was equipped with an electric powered pumping unit. There was a partial dike around the equipment, but not enough to be effective at containing fluid. Correct, legible identification was posted at the wellhead and tank battery. There were no leaks or spills at the tank battery.
