0333299700 2016-10-19 Production Wells No Violations On the day of my inspection I found that the well was shut in. The flow line off of the tubing was connected. The casing line was disconnected. The valves were closed. I called Mark Fluharty to ask about their plans for the well. He claimed that he would try to get his swab rig back in there by the end of the week.
0828770309 2015-10-14 Production Wells Violations Noted On the day of my inspection I found that the wellhead was equipped with tubing and a swage. The tubing was disconnected from the flow line. The tank battery is shared with permit 2997 and consisted of two 100 barrel tanks, one separator and an earthen dike. There were not separate charts to show individual production for each well. The reported production is identical to that of permit 2997. In its current state this well is not equipped for production. The well owner/operator shall submit a written plugging schedule to Chris Grimm at 5880 Memory Rd, Zanesville, OH 43701 within 30 days, per OAC 1501:9-11-05(B), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied.
1069719318 2017-09-25 Plug / Plug Back No Violations Mark Fluharty called me early in the morning to report that Derek Buxton could not get a telegraph until 306 feet. I told Mark to go ahead and try to rip there, but they needed to make a good effort. When I arrived at the well site I found that Derek was trying to pull the casing. He said that they got it knocked off at 306 feet and they got it to come up the hole about two feet when it just locked up on them. They made multiple attempts to pull the casing and it wouldnt move. I told Derek to come up to the next collar and try to rip it again. They came up the hole and found another faint telegraph at 284 feet. They ripped the casing again and tried to pull with no success. I had Derek pull on the casing to where he was comfortable with the weight and lock it in for a while. They continued to pull on the casing and it did not budge. I told Derek to keep coming up until he gets something out. They got the tools to set down at 140 feet and ripped the casing for a while before trying to pull. The casing still would not budge. They came up to 100 feet and had the same result. I finally told Derek to come up to a point where he gets a good telegraph and get out what he can. I did stress the need to at least get one or two of the top joints out. I also said that they could try unscrewing the top joint. They did end up getting the pipe to come free at 57 feet.
1288292338 2017-09-28 Plug / Plug Back No Violations On the day of my inspection I found that RB Drilling had rigged down and moved out. All of the plugging equipment, materials and debris had been removed from the site. The pit was closed and the site was graded out. I spoke with Mark Fluharty the morning after the well was plugged to surface, and he said the cement top was down 11.5 feet. Mark sent me photographs of the casing being cut off 36 inches below grade, and photos of the steel plate that was inscribed with the permit number. This well had been plugged and abandoned. The compliance notice has been resolved.
1449593669 2015-12-08 Administrative Inspection No Violations I received written notification of intent to put this well back into production. The letter states that Northwood Energy would like to reestablish the sales line connection at the tank battery and pressure test the flow line. Once tested they intend to turn the well on and run sales through the same meter as the Shawver 1. I spoke with Cory McCudden, geologist with Northwood, and discussed a time frame. The work shall be completed and the well producing prior to 4/14/16.
1461496366 2018-06-07 Final Restoration No Violations On the day of my inspection I found that the access road and well site were both final graded and approximately 90 percent covered in tall grasses and broadleaf vegetation. There were no oilfield related materials or debris left at the site. I did not see any signs that the plugged well was leaking. The production equipment is still in use for permit 2997. This site passes final restoration.
1463751525 2016-05-20 Production Wells No Violations On the day of my inspection Mark Fluharty was rigged up to swab the well. There was very little gas pressure built up when he opened the well. He was swabbing through 1.5 tubing that was open ended with a seating nipple. The fluid level was at approximately 1500. Mark made multiple pulls over the course of four hours. He swabbed approximately 96 barrels of fluid from the well. The ratio of oil to water was unknown. There were no gas shows during the swabbing. After four hours Mark was at the bottom of the tubing and was pulling very little fluid. The well went in vacuum. Bill Arnholt instructed Mark to shut the well in and let it set for 7 days. He is scheduled to swab again on Friday, May 27.
1464372167 2016-05-27 Production Wells No Violations On the day of my inspection I witnessed Northwood Energy Corp. has a swab rig over the well bore in attempts to produce oil to make commercial production requirements. Over the duration of the day I witnessed approximately five bbl. of fluid was swabbed out of the well. I collected a sample of the fluid during the first pull and discovered most of the fluid was brine (approximately 3% oil and 97% brine). I witnessed this operation today as a request by Inspector Chris Grimm and will transfer the information to him.
1464965085 2016-06-03 Production Wells No Violations On the day of my inspection I met Mark Fluharty at the well site to witness the well being swabbed. This was the third time the well had been swabbed in two weeks. He estimated the fluid level to be at 2200 on his first pull. Mark made 15 runs and pulled approximately 400 gallons of fluid. The ratio of oil to water was unknown.
1478111014 2016-11-02 Production Wells No Violations On the day of my inspection I found that Mark Fluharty was rigged up to swab the well again. The well was swabbed three times in late May to early June 2016. Northwood claimed that they swabbed approximately 6 barrels of oil from the well once the water was separated. The well has not been swabbed since 6/3/16. There was an insignificant amount of gas when Mark opened the well. He estimated the fluid level to be at a depth 2400 feet on his first pull. Mark made 10 runs and pulled approximately 200 gallons of fluid. The fluid that was collected in a bucket from a bleeder valve during each pull was mostly salt water, but the exact ratio of oil to water was unknown. Northwood is trying to avoid plugging this well by swabbing the minimum amount (15 barrels) of annual oil production from the well. They have not met that goal. I have maintained my position with Mark Fluharty that the well needs to be plugged. He said that he would discuss the situation with Bill Arnholt and get back to me.
1506007957 2017-09-21 Plug / Plug Back No Violations On the day of my inspection RB Drilling lined the pit. The 1.5 inch diameter tubing was set at a depth of 3315 feet, open ended. Formation hooked up to the tubing and pumped to break circulation. The pressure went up to 800 pounds and Formation shut down. The pressure was bleeding off very slowly. They tried to pump again and the pressure went up again. RB Drilling hooked up to pull one joint and the tubing pulled freely. They laid one joint down and Formation went back to pumping. The pressure stayed around 400 - 600 pounds once they broke circulation. Formation circulated all of the oil out of the hole and went to mixing the cement. They pumped the plug and displaced with no significant fluctuations in pressure. Circulation was maintained throughout the job. RB Drilling pulled the tubing from the well. Appalachian Well Surveys came in and shot the 2.875 inch diameter casing off at a depth of 2650 feet. Derek Buxton pulled and the casing was free. They started pulling the 2.875 inch casing out of the well.
1506097540 2017-09-22 Plug / Plug Back No Violations On the day of my inspection I found that Derek Buxton had pulled all 2650 feet of the 2.875 inch casing from the well. They ran the 1.5 inch tubing back in the well to a depth of 2600 feet to set a Big Lime plug. Formation pumped until they started moving fluid and then started mixing cement. The well circulated while they were mixing. They pumped a 250 foot plug in order to cover the Oriskany sand, because there was a show indicated on the well card. RB Drilling pulled the tubing up to 960 feet. Formation established circulation and pumped a 150 foot cement plug through the Berea sand. Mark Fluharty called me back to the site later in the day to witness them trying to pull the surface casing. They did make several attempts that appeared to be pushing the rig to its limits, and the casing did not budge. I told Derek to go ahead and try to rip the casing off near the bottom few joints. They went in and got a faint telegraph at 426 feet. They tried ripping for a while when the knife broke in the tools. Derek shut down for the weekend.
1506443241 2017-09-26 Plug / Plug Back No Violations On the day of my inspection I found that Derek Buxton had pulled the 57 feet of surface casing from the well. They ran the tubing back in to a depth of 564 feet for the plug at the base of the surface casing. Formation established circulation and pumped a 300 foot cement plug to cover the base of the surface casing and the two lower depths where the casing was ripped at 306 feet and 284 feet. RB Drilling pulled all of the tubing from the well and the cement was left to set up for two hours. Then RB Drilling ran back in with the tubing to a depth of 166 feet for the surface plug. Formation pumped until cement was circulated to the surface. The cement fell approximately 20 feet once they stopped pumping and the tubing was pulled out. Formation topped the cement off a few times until it stayed up at the surface.
1574702533 2017-05-08 Production Wells No Violations On the day of my inspection I found that the well was shut in and the flow line was disconnected. There was no identification posted at the wellhead. The access road was blocked to vehicle traffic with a fallen tree. There were no leaks at the wellhead. I called pumper Mark Fluharty to ask what the plan was. He did not have an answer, so I called Bill Arnholt with Northwood Energy. He did not answer so I left a voice message.
1729758911 2017-09-20 Plug / Plug Back No Violations On the day of my inspection I found that the pit had been dug, but not lined. There were no sharp rocks or roots visible in the bottom of the pit. RB Drilling had moved in and rigged up on the well. There was a 100 barrel steel tank set back at the well site that was full of water. Mark Fluharty notified me that cementers were scheduled for 09:00 the following morning to set the bottom plug.
2096503719 2016-05-16 Production Wells No Violations On the day of my inspection I found that a large tree had fallen across the lease road, preventing vehicle access to the well. I walked back to the well and found that it was equipped with a 2.875 wellhead and 1.5 tubing. The surface casing was buried and not visible. The nipples on the wellhead were shut in. The tubing was equipped with a ball valve and an elbow connector that swaged down to a 1 valve and 1 flexible, rubber hose that connected to the flow line through one more valve. All three valves were closed. There were no gauges on the well to indicate pressure. I called Bill Arnholt with Northwood Energy to ask about the well. He claimed that they attempted to swab the well through the 2.875 casing back in January of 2016. The swab tools got hung somewhere around 2500 and the sand line parted. Arnholt reported that they lost 800 of sand line and the swab tools down the well. They brought in a service rig in an attempt to fish the tools out. The rig recovered 300 of sand line and gave up. Then they ran 1.5 tubing to 3315 and shut the well in. Bill claimed that he believed they could still produce the well as a swab well even though the tools were lost down hole. I told him that I want to be out there when they swab and I requested that they have the well swabbed as soon as possible. I questioned Bill as to why the well had been shut in for so long and I asked if there is a hole in the casing. He claimed that he didnt believe that there was a hole in the casing and he didnt know why the well had been shut in for so long. I also questioned him as to why the lost tools were never reported until now, but I didnt get an answer.
2118920893 2017-09-19 Production Wells No Violations I got a phone call from Bill Arnholt with Northwood Energy to notify me that they were starting to move in to plug the well. I told Bill that I had not received a plugging permit yet. He said that he would call central office to check on it. He called me back and said that he and spoke with Steve Opritza, who allegedly told him the permit would be issued on 9/20/17. We also discussed the proposed plug depths. On the day of my inspection I found that Mark Fluharty with Northwood Energy had cleared the access road back to the well. The site had also been cleared of brush. There was a dozer and track-hoe on site. The pit had not been dug yet. Bill said that Derek Buxton with RB Drilling would be moving in tomorrow, and they wanted to set a bottom plug on Thursday. I called Mark Fluharty who said that he would be out tomorrow morning to dig the pit get the rig in to the site.
