0172325193 2017-07-19 Production Wells No Violations On the day of my inspection I found that the field surrounding the well and tank was growing in soybeans. The area where I had suspected brine scald, and that Carrie Moore allegedly treated was bare with no crops growing. The alleged lime and fertilizer treatment did not work. I called Carrie Moore and Rick Pace and left messages for them to get back to me. Rick Pace did return my call and I told him that they can try one more time to treat the soil and show that it can support grass growth. I suggested bringing in some new top soil and mixing it into the affected area along with lime and fertilizer. The other option that I suggested was to excavate the affected soil and replace it with new top soil. I told him that the compliance notice will remain open until they can demonstrate vegetative growth.
0208475820 2017-10-20 Production Wells No Violations On the day of my inspection I found that the area that had been impacted with brine water was still completely bare. There were some new tractor tracks at the access road entrance. The impacted soil looked as though it may have been stirred up a little, but there were no definite signs of treatment or tilling. The soil was bare, compacted and there was no seed or straw present. I asked my district supervisor Mike Moreland for assistance with contacting Carrie Moore and pressuring her to properly remediate this site.

On the day of my inspection I found that nothing had been done to the soil in the bare area below the tank. There were no equipment tracks and the soil did not show any signs of recent disturbance. I spoke with Carrie and she claimed that she was coming out today with a tractor and plow, and she was going to work fertilizer and lime into the soil.

0363355744 2018-11-07 Production Wells No Violations On the day of my inspection I found that nothing had been done to remediate either brine scald area at the well site. The scald behind the tank was still 90% bare, with some very sparse grasses around the perimeter. The scald in front of the wellhead was still a bare, muddy patch that was filled with deer tracks and standing rain water. I tested the water with a high range Quantab and found the chloride level to be less than 250 parts per million. The access road was starting to wash out worse than my last inspection.
0408566739 2017-03-20 Production Wells No Violations On the day of my inspection I found that the well was equipped with an electric powered pumping unit. The electric was connected an the drive belts were in place. The flow lines were connected and the valves were open. The tank battery consisted of one 100 barrel steel tank and one horizontal separator. The tank was new and unpainted. There was not an earthen dike around the production equipment. Correct, legible identification was posted at the pumping unit, which was in plain view of the tank. The soil behind the tank was bare leading downhill. The bare muddy soil was heavily tracked by deer. Carrie Moore and one of her pumpers showed up while I was at the well site. I questioned her about the bare area, pointing out the physical signs that may indicate a brine release. She denied that the bare area was a result of a brine release and alleged that it was from flow line repairs. I waited until they left to conduct a field test for chlorides, but I was out of Quantabs. I will return as soon as possible to further investigate the area.

0444300934 2017-10-31 Production Wells No Violations On the day of my inspection I met Carrie Moore at the site per her request to field test the bare soil for a rough estimate of the chloride level as compared to my first inspection on 3/22/17. I found that the chloride level was at 1112 parts per million (ppm) as compared to 1037 ppm back in March. There has been no reduction in the chloride content of the soil. Carrie agreed that the site was worse than she thought and she claimed that she would get someone out here to start working the soil. I pulled up past satellite imagery on Google Earth which showed the brine scald being present in September, 2014. Carrie took ownership of the well in August of 2014 and she said that the tank was changed out shortly after. The next historical image was taken in May, 2012 and the scald was not shown. We agreed to that she will work to remediate the soil through the fall and early spring, and we will make an assessment when crops are planted again in May, 2018.

1030630864 2017-03-22 Production Wells Violations Noted On the day of my inspection I returned to the well site to complete my investigation into the bare area that I noticed on 3/20/17. I found that there was a smaller darkened bare spot in the front of the tank near the load valve. The valve was equipped with a bull-plug, but there was not a lock on the handle. The tank was in good repair and it was empty. There were no indications that it had been leaking or run over. The darker coloration in the soil continued around the tank, following the natural grade to the downhill slope, where it opened up to a triangular shaped bare area that was heavily tracked by deer. The area was approximately 50 feet in length on all sides. I collected three tablespoon size (1/2 oz.) soil samples from the top 2 inches of the surface. I mixed the soil with approximately 2 tablespoons of dh2o and tested the soil/water mixture with a low range Quantab. The chloride indicator strip maxed out at 10 units (644 parts per million) after titration. I then tested the same sample with a high range Quantab, which indicated 4.8 units (1037 parts per million). I called Carrie Moore to notify her that I was issuing a compliance notice. She claimed that she had no knowledge of a brine release at this site. All of the brine impacted soil must be removed from the site and disposed of. The affected area should be treated with lime and fertilizer. Vegetative growth must be established on the affected area to prevent erosion. It is also recommended that an earthen dike be constructed around the production equipment.

1220227622 2019-04-16 Production Wells No Violations On the day of my inspection I was accompanied to the well site by SE Regional Supervisor Brent Bear. We found that the well was equipped with an electric powered pumping unit. The motor, drive belts and electric line were intact, but the electric switch was in the off position. The flow lines were connected at the wellhead. The tubing valves were closed. One casing valve was closed, and one was partially open to the solvent drip bottle. There was a 24 inch diameter circle of oil and paraffin on the ground around the wellhead, and the brine scald area behind the production tank has never been remediated. This well has an outstanding compliance notice for pollution and contamination, with a remedy date of 4/22/17. I took updated photographs of the wellhead.

1265064201 2017-04-12 Production Wells No Violations I was contacted by Rick Pace, pumper for KMFAD to request that I meet him at the well to go over the issues and discuss corrective actions. We walked around the site to assess the extent of the impact. I told Rick that the brine scald area will need to be remediated by scraping up the affected soil, treating the site with lime and fertilizer, spreading new clean top soil and seeding the area for erosion control. I also suggested that they construct an earthen dike around the production equipment. Rick was working on the dump valve. He said that he would talk to Carrie Moore about getting the resources to start the clean-up.

I called Rick Pace, pumper for KMFAD. He said that Carrie spread lime and fertilizer on the area, but nothing else has been done. 1533220741 2018-08-02 Production Wells No Violations On the day of my inspection I found that nothing had been done to remediate the brine scald areas behind the tank and in front of the wellhead. The field surrounding the well site was grown up in tall corn, but the area behind the tank was still mostly bare. There was more crop growth around the perimeter of the brine scald than I observed last growing season, but the majority of the area was still demonstrating an inability to sustain plant growth. The brine scald area in front of the wellhead was completely bare and heavily tracked by deer. There was some rain water laying in a vehicle track that I tested for chlorides with a low range Quantab. The test strip showed 0.079% NaCl, or 477 ppm(mg/L) chlorides. I called Carrie Moore and I got no answer. I called the pumper Rick Pace and he said that he hasnt heard anything from Carrie on getting the site remediated. Rick said that he has a roto tiller that he will try to bring out to turn the soil and treat it with lime and fertilizer.
1534873458 2018-08-21 Production Wells No Violations On the day of my inspection I found that nothing had been done to remediate the two brine impact areas at the well site. .I have not been able to get any communication from Carrie Moore. I called the pumper, Rick Pace the day of my last visit and he agreed to bring a tiller out to churn and treat the affected soil. The area behind the tank, in the corn field is still bare and some small erosion ditches are starting to form. The area in front of the wellhead was full of water from heavy rain in the past 12 hours. I sampled the water with a Quantab strip and found that the chloride level was 492 parts per million. I called Carrie Moore again and I got no answer, so I left another message.

1594325841 2018-06-01 Production Wells No Violations Carrie Moore, owner of KMFAD Oil and Gas, contacted me yesterday to notify me that she was on her way to this site to begin remediation on the bare soil that had been affected by brine water. On the day of my inspection I found that nothing had been done. There were no signs that any of the brine scald area had been treated or tilled in any way. There were planting rows cut into the soil where the farmer has been out to work the field. The entire field and the immediate area around the pumping unit and production equipment was growing in waist high rye and orchard grasses, except the scald area, which was still completely bare. There was also a shallow erosion channel starting to form in the downhill direction. In addition to the brine scald behind the tank, I found a new area of concern directly in front of the wellhead where the vegetation had all died. There were oil and paraffin stains in the dead grasses, and the bare muddy spots were heavily tracked by deer. There was no standing fluid to get a Quantab reading. There was also oil on the gravel around the wellhead. I called Carrie to ask what she was doing and I got no answer. I sent Carrie a text message about this well and she did not respond.
1648506471 2018-05-22 Production Wells No Violations On the day of my inspection I found that there were no signs that any work had been done to remediate the brine impacted soil behind the tank. The field surrounding the well site and production equipment was grown up with a wheat or rye cover crop. The impacted area was still 100 percent bare, with no weeds, grasses or crop vegetation present. There were some small erosion channels forming in the soil and the bare area was starting to extend outward into the crops more than what I found on my initial inspection. Carrie Moore had assured me that she would have the site tilled and treated prior to the 2018 planting season. I called Carrie and she did not answer. I also called the pumper Rick Pace and he did not answer either.

I called Carrie Moore and she claimed that they had a contractor lined up to come out next week.
