0764651118 2014-06-26 Administrative Inspection No Violations Follow up to CA #1396975710: This inspection correlates with complaint #4654. I contacted Carrie Moore, who recently acquired ASNF Producing, in regards to the status of this well. She claimed that they are moving a service rig onto the well within the next week to assess whether they can put the well back into production or plug it.
1341908060 2015-04-30 Final Restoration No Violations On the day of my inspection I found that all of the production materials and debris had been removed from the site. The lease road was still in place. I called Carrie Moore to request that she either restore the road or get a landowner waiver signed to leave it.
1396975710 2014-04-07 Production Wells Violations Noted I received a call from Mr. Jim Buxton in regards to a non-producing oil and gas well on his property. I met Mr. Buxton at the location to inspect the well. On the day of my inspection I found that the well was equipped for a pumping unit, although the pumping unit had been removed. There was a stuffing box and polished rod protruding from the well head. The valves were closed. The tank battery consisted of one 100 barrel tank and one separator. Identification was posted and I found no leaks or spills. There has been no reported production for this well since 2004. This well must be plugged within 180 days from the date of compliance notice issue.
1412856138 2014-10-08 Administrative Inspection No Violations I contacted Mark Schubert, the field supervisor for KMFAD, in regards to plugging this well. He claimed that Moore Well Service was rigged up on the well to pull tubing. They were going to have to strip the well and then determine if it needs plugged or they can put it back in production.
1413998841 2014-10-21 Plug / Plug Back No Violations On the day of my inspection Moore Well Service pulled the 2.875 casing up to 2512 Petroset broke circulation and pumped a 220 cement plug at the top of the Big Lime. Moore Well Service pulled the casing up to 1024 and Petroset pumped a 167 cement plug through the Berea. Moore pulled the remaining casing from the well.
1414073506 2014-10-23 Plug / Plug Back No Violations I met ODNR inspector 3 Pat Shreve at the location to evaluate the situation. Moore Well Service attempted to unscrew the top joint of the 8.625 casing with a bulldozer winch-line. The casing would not budge. The decision was made to perforate the casing and cement it to surface. Appalachian Well Surveys shot the casing with 10 shots, spaced at 1 foot intervals, at 350, 250, and 150. Moore Well Service ran tubing to 544. Petroset broke circulation and pumped a 250 plug. Moore pulled the tubing up to 294. Petroset broke circulation and pumped until cement was circulated to surface. The cement fell approximately 150 when they stopped pumping. They dumped 100 lbs. of cotton seed hulls down the casing and topped it off with cement. The cement did not fall.
1414163305 2014-10-24 Plug / Plug Back No Violations Follow up to CA #1396975710: This inspection correlates with complaint #4654. On the day of my inspection I found that the cement had fallen approximately 10. Moore Well Service cut the casing off 36 below grade and capped it with a steel plate inscribed with the permit number. This well is plugged and abandoned. This violation and complaint is resolved.

1845722674 2014-10-14 Production Wells No Violations On the day of my inspection I met Mark Schubert at the site. Moore Well Service was rigged up swabbing the well. They had pulled the rods and tubing. Mark reported that they attempted to swab the 2.875 casing, but they couldnt get their tools down the casing due to paraffin build up. They ran the 1.5 tubing back in the well to 3449 and were swabbing through the tubing. The fluid was down approximately 1500. They were bringing oil up with the swab. The fluid weighed 7.8 pounds per gallon. The goal is to swab all of the fluid out of the well to determine if the well is capable of gas production.
1925994042 2014-10-22 Plug / Plug Back No Violations On the day of my inspection Moore Well Service attempted to pull the 8.625 casing. They made several attempts up to 60000 lbs. and the casing would not come free. They went in with a ripping tool to 440 and searched for a telegraph. They did not get a good telegraph until 70. They ripped holes in the casing to the point where the ripping tool was getting stuck. They came up to 53 with the same result and they continued to pull 55000 to 60000 lbs. on the pipe with no success. They also tried to unscrew the top joint with power tongs up to 2500 lbs. of torque. The pipe would not budge.
