0047343177 2015-12-09 Production Wells No Violations On the day of my inspection I found that MFC had made a location to access the well. I spoke with Jeff Harmon. He claimed that they wanted to swab test the well to determine if they can put it back in production. I spoke with Jim Aslinides and told him that the well needs to be plugged. I was told that I do not have the authority to deny them the opportunity to test the well and attempt to put it back in production.
0360924481 2016-02-29 Production Wells No Violations On the day of my inspection I found that the well was still shut in and the line was not connected. I called Jeff Harmon with MFC to get an update on the plans fro the well. He claimed that their service rig was still tied up at a fishing job, and the plan was to move to this well as soon as that job was completed.
0409848230 2016-03-21 Plug / Plug Back No Violations On the day of my inspection I found that MFC Drilling had moved their service rig onto the location in order to plug the well. I met with Jim Moore of MFC, who said that the well had 700 psi of gas pressure last week when he blew it down and it was back up to 400 psi from sitting over the weekend. MFC plans to swab any remaining fluid out of the 4.5 casing in order to bull-head the bottom plug. Jim claims that there is no indication of a hole in the casing.
0492131364 2015-12-22 Production Wells No Violations On the day of my inspection I found that MFC had not moved in to start work on the well. I called Jeff Harmon. He said that they were waiting on the location to dry out or freeze up, and they would probably be moving in after the holidays.
0959519465 2015-04-17 Production Wells Violations Noted On the day of my inspection I found that the 8 wellhead was exposed. The 4.5 casing was equipped with a swage that was connected to 2 line with a series of valves, terminating with 1 plastic gas line leading back into the ground. All of the valves were closed and the gas line was cut in half. I found the well in the middle of a wet forested strip between the edge of an agricultural field and county road. There were no foot or vehicle tracks leading to the well. I found a disconnected 2 flow line protruding from the ground near the wellhead. I found no tanks or production equipment for the well. There was no identification posted at the wellhead. The well was not leaking. The only production that has ever been reported for this well was 100 mcf gas in 2010. This well must be plugged within 180 days of the issuance of this compliance notice.
1348712093 2016-07-15 Final Restoration No Violations On the day of my inspection I found that the well site was approximately 85% covered in grasses and broadleaf vegetation. I found no signs of erosion. There was no tank battery or production equipment associated with this well. I found no oilfield related materials or debris at the site. There was no access road for this well. The location is directly off of CR 271. This site passes final restoration.
1445358764 2015-10-20 Production Wells No Violations On the day of my inspection I found that there had been no change in the status of this well. It was still shut in and disconnected. There was still no passable access to the wellhead. I found no identification. The well was not leaking. There was a plugging permit issued on 10/2/15. I called MFC and spoke with Jackie Wilkins. I explained to her that they were required to plug the well by 10/17/15, and that getting a plugging permit does not give them another two years to comply. She claimed that she would get the message to Jim Aslanides.
1452019848 2016-01-05 Production Wells No Violations On the day of my inspection I found that the service rig had not moved in yet. I spoke with Jim Aslinides who claimed that they had a vandalism incident in Ashtabula Co. that sidetracked their rig, and they would be moving in as soon as they get it resolved.

1458822586 2016-03-23 Plug / Plug Back No Violations On the day of my inspection MFC Drilling and Cemenco plugged this well. ODNR Inspector Randy Akins witnessed the bottom plug and the big lime plug. He reported that they bull-headed the bottom plug, and ripped the 4.5 casing off at 3040. Randy reported that circulation was established prior to pumping the big lime plug. When I arrived on the location they were ready to pump the Berea plug. Circulation was established and maintained while pumping. MFC and Cemenco also set cement plugs at the bottom of the surface casing and from 200 to surface. Circulation was established prior to pumping both plugs and cement was circulated to surface on the final plug. The cement fell approximately 20 feet when MFC finished pulling the 4.5 casing. Cemenco topped it off and the cement stayed up. MFC said that they would leave the well open overnight to check the cement level in the morning. This well has been plugged and abandoned. The compliance notice is resolved.
1665996753 2016-01-29 Production Wells No Violations On the day of my inspection I found that the well was still idle and incapable of production. I called Jeff Harmon with MFC and left a message to contact me in regards to their plans for this well.
1784303955 2016-03-30 Plug / Plug Back No Violations On the day of my inspection I found that the well casing had been cut and buried, and the site had been graded out. All of the plugging materials and debris had been removed from the location. I called Jim Moore, rig operator for MFC to ask about the cement. He claimed that the cement only fell 5 feet after sitting open overnight. He said that he cut the casing off 3 feet below grade and capped it with a steel plate.
1957376192 2016-05-19 Final Restoration No Violations On the day of my inspection I found that the site had not been seeded yet and no vegetation was growing. There were no erosion problems at the site.
