0210848793 | 2018-01-10 | Production Wells | No Violations | On the day of my inspection I found that the pumping unit was in operation. There were no leaks from the wellhead components. Guy Dagan and Kevin Slaughter were at the production equipment site working on the under tank heaters. The flare stack was burning when I pulled up. The identification was still legible on the tanks. There were no leaks or spills around the production equipment. |
0326339326 | 2018-09-13 | Production Wells | No Violations | On the day of my inspection I found that the pumping unit was upright and the wellhead components were reconnected. Dugan was starting to pull the engines off of the pumping units in preparation for winter. The wells on this lease are located in an area that is prone to flooding. |
0538782118 | 2018-02-20 | Production Wells | No Violations | On the day of my inspection I drove out past the Mohawk Dam to check on the two wells that had been damaged in the last flood. The water was so high that I could not actually get close to t he wells, they were completely under water. I called Barry Dugan and he said that the wells should not be taking water because there is no way for it to get in the production casing as long as the wellhead is intact, and there should be no way for it to get into the producing formation s long as the downhole pump is still seated. Barry said that they probably wont be able to get a rig in to work on the wells until June because the area is subjected to frequent flooding throughout the winter and spring months. |
0640606178 | 2018-02-12 | Production Wells | No Violations | On the day of my inspection I found that the pumping unit was laying on its side. The polished rod was bent and the tubing was broke off at the tubing head. There was no fluid or gas leaking from the well and I found no oil on the ground around the wellhead. The engine was still mounted to the unit and the gearbox cover had fallen off. I didnt see any motor oil or transmission fluid on the ground under the unit. Barry Dugan said that he had two units overturned recently when the rivers flooded and an ice dam broke loose. He said that they have everything shut in and they plan to get a rig on the well as soon as the weather permits. |
0747770482 | 2018-03-20 | Production Wells | No Violations | On the day of my inspection I found that the flood waters behind the Mohawk Dam had subsided, allowing access to the area. The pumping unit was still on its side and there was no further damage to the wellhead components. I found no oil on the ground around the wellhead, and I did not find any gas leaking from the wellhead. There were no visual indications that being under water for an extended amount of time had caused any leaks from the wellhead. The production equipment appeared to be unaffected by the flood waters. There was a visible water line in the trees throughout the area, and the tanks are located in an elevated position that was well above the water line. I did not find any debris in or around the dikes, and I found no water lines on the tanks. There were no leaks or spills around the tanks. |
0790134249 | 2016-11-01 | Production Wells | No Violations | On the day of my inspection I found that the contaminated soil around the wellhead and under the engine had been cleaned up. I found no active leaking from the stuffing box or any of the other wellhead components. I also found no active leaking from the motor or gearbox. |
1017199519 | 2007-12-19 | Production Wells | No Violations | Motor removed for winter , well is behind Mohawk dam . |
1622341773 | 2016-10-05 | Production Wells | No Violations | On the day of my inspection I found that the well was equipped with a natural gas powered pumping unit. The engine and drive belts were intact. The flow lines were connected and the valves were open. There was a small area of oil stained soil around the wellhead from a flow line leak. There was also oil stained soil under the engine from a fluid leak. There was identification posted at the wellhead. I verbally notified Guy Dugan of the issues that need to be addressed at the wellhead. |
