0962043027 2012-11-30 Production Wells Violations Noted I received an idle well complaint from landowner Rod Goedel on 11/28/12 concerning two wells on his property. On the day of my inspection I met with Mr. Goedel to look at the wells. I found that the well head was equipped with a swedge and shut in. There was a flow line to the tank battery which consisted of one 100 barrel stock tank and one separator. There was no meter or chart present and the separator was overgrown with vegetation and in a poor state of repair. Proper identification was posted. I have a current plugging permit for this well that was issued to MFC Drilling on 2/21/12. The well changed ownership to Elephant Exploration on 11/26/12. The well needs to show valid production or be plugged.
0997359086 2013-01-17 Production Wells Violations Noted I was conducting a follow up inspection for compliance agreement 962043027 when I discovered that the gravel around the bottom of the stock tank was saturated with oil. Upon closer inspection I found that the crude oil had drained across the lease road, into the adjacent corn field and meandered approximately 50 meters to a small tributary of a stream/agricultural drainage ditch. There was visual evidence of contamination at the point where the tributary was fed from the field and behind a clump of vegetative debris at the mouth where it fed the stream. I sampled the water upstream and downstream of the tributary. I found that both samples tested negative for chlorides and both samples had a pH of 6. The stream was flowing rapidly and showed no signs of contamination. I inspected the culvert at SR 93 and found no visual evidence of contamination. I advised Elephant Exploration that they need to get absorbent booms and diapers in the tributary, to spread peat moss on the areas of contamination, and ALL contaminated soil must be removed from the site. Additionally, I am requiring that the tank battery for this well be diked.
1358950158 2013-01-23 Production Wells No Violations On the day of my inspection I found that all of the contaminated soil and gravel had been removed from the location. An earthen dike with a new gravel base had been constructed around the tank battery. The oil in the tributary had been removed with absorbent pads. The absorbent boom was left in place to collect any residual contamination.
1366117846 2013-04-16 Production Wells No Violations FOLLOW UP TO CA # -962043027: On the day of my inspection I found that the well head was equipped as a swab well. The lines to the tank battery were connected. There was a newly constructed dike around the tank battery and proper identification was posted. The annual production report for 2012 showed that 15 barrels of crude oil were swabbed from this well, which satisfies the minimum requirement for viable production. This violation is resolved.
