0139351116 2016-06-01 Plug / Plug Back No Violations On the day of my inspection Gene Matheny washed 2 tons of clay down the 4.5 casing. They ripped the casing at 2570, pulled 19 joints and washed another 3 tons of clay down the 4.5 casing for a total of 5 tons on the bottom plug. Matheny pulled the remaining 4.5 casing from the well. Then he set a brush and stone bridge at 2140 for a clay plug at the top of the Big Lime. Matheny tagged the bridge to ensure proper placement and then washed 3 tons of clay on top of the bridge for a 200 plug.
0617880845 2016-05-31 Plug / Plug Back No Violations On the day of my inspection I found that Gene Matheny was rigged up to plug the well. The rods and tubing had been pulled and the wellhead was removed. I met with Gene to discuss plugging details and method. He will be plugging with prepared clay. We also discussed approved clay sources. Gene said that he was getting the clay from Logan Clay.
0985582349 2016-11-28 Final Restoration No Violations On the day of my inspection I found that the well site and part of the access road had been completely reclaimed into agricultural field. The site was graded flat and covered in soybean stubble. There were no oilfield related materials or debris left behind. The production equipment and remaining access road is still in use for permits 2154 and 2499. This site passes final restoration.
1464875340 2016-06-02 Plug / Plug Back No Violations On the day of my inspection Gene Matheny set a pre-cast cement plug on top of the 7 seat at 800. He washed 3 tons of clay down the well and pulled all 300 of the 8.625 casing. Once the surface casing had been pulled Matheny washed another 3 tons in and the well filled to surface. They let the clay settle for one hour while they rigged down. It fell approximately 50 and they got 1 more ton washed in before the well was filled to surface. We watched it for another hour and the clay stayed at the surface. The conductor was already more than 36 below grade, so Matheny capped it with a steel plate inscribed with the permit number.
