0051072218 2014-05-21 Final Restoration No Violations On the day of my inspection I found that the well site was graded out to the approximate original contour. There was approximately 80% well established grass coverage. I found no evidence of erosion. The lease road and tank battery is still in use for permit 1899. This site passes final restoration.
0325335969 2013-02-07 Plug / Plug Back No Violations On the day of my inspection I found that the service rig and all related equipment had been removed from the site, and the soil was graded out to the approximate original contour. The lease road will remain in place at this site, as it is used to access the tank battery for permit 1899.
0421471943 2013-01-24 Plug / Plug Back No Violations I was given 48 hours notification by Mike Wheeler that plugging operations would begin on permit #1899. On the day of my inspection I found that the service rig was rigged up on permit #1960. After some dispute and a thorough investigation into the well records, plat maps and GPS coordinates I determined that the crew was preparing to plug permit #1960. They did not have a valid plugging permit for this well and were not permitted to proceed with any further plugging operations. I contacted the Mason Producing and Erica Freeman in ODNR permitting to get the situation straightened out. The crew was shut down for the day. The rods and tubing had already been pulled; no clay was put into the well and the production casing had not been ripped.
0917943110 2013-01-31 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler shoveled an additional 1.5 tons of clay for a total of 5 tons across the Berea. The crew then pulled 448 of 8.625 casing from the well. They set a pre-cast cement plug at 448 and shoveled 9.5 tons of clay.
1359467766 2013-01-28 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler shoveled 5 tons of clay into the 5.5 casing for an approximate 512 plug from 3192 to 2680. The crew then ripped the 5.5 casing at 2230 and began pulling the casing from the well.
1359486538 2013-01-29 Plug / Plug Back No Violations On the day of my inspection Mike Wheeler pulled 2230 of 5.5 casing from the well. A pre-cast cement plug was set on top of the 5.5 casing at 2230 and 3 tons of clay were shoveled to provide a 150 plug through the top of the Big Lime. A brush and stone bridge was set at 950 and 3.5 tons of clay was shoveled through the Berea.
1475770933 2013-02-01 Administrative Inspection No Violations Mike Wheeler reported that they shoveled another 5 tons of clay to fill the well to surface. They cut the casing off 36 below grade, capped it with a plate, moved the service rig off of the location and graded the soil out to the original contour.
2133435981 2013-01-25 Administrative Inspection No Violations I received notification from Erica Freeman in ODNR permitting that the situation had been sorted out with the well owners and a corrected permit would be issued shortly. I received an electronic copy of the corrected permit. I called Mike Wheeler to notify him that I had a valid plugging permit for well #1960 and that he was cleared to resume plugging operations.
