0494225373 2019-05-14 Final Restoration No Violations On the day of my inspection I found that the well site was graded out, but it had not been seeded and there was very little vegetative growth. There was also one 100 barrel steel tank left at the site. I called MFC to remind them of what still needs done to complete the final restoration.
0536314947 2018-05-21 Production Wells No Violations I was contacted by landowner Dave Foster to request an inspection on this well because he is thinking about buying the well for domestic use. On the day of my inspection I found that the well was equipped with production casing and a swage. The flow line was disconnected and the valve on the swage was closed. Mr. Foster claimed that the line has been disconnected for at least 10 years. The production equipment consisted of one 100 barrel steel tank in a shallow earthen dike. The tank was empty and the bottom was starting to rust. Correct, legible identification was posted on the tank and there were no leaks around the wellhead or tank.
0924985206 2018-10-01 Production Wells No Violations On the day of my inspection I found that the well was equipped with production casing and a swage. The flow line was disconnected and the valve on the swage was closed. The landowner, Mr. Foster claimed that the line has been disconnected for at least 10 years. The production equipment consisted of one 100 barrel steel tank in a shallow earthen dike. The tank was empty and the bottom was starting to rust. Correct, legible identification was posted on the tank and there were no leaks around the wellhead or tank. The annual production reports show 373 thousand cubic feet of gas in 2017. I called Jim Aslinides of MFC to ask about the well. He did not answer so I left a voice message. Jim returned my call and claimed that he thought the landowner might want the well as a supplemental heating source. I told Jim that the well looked idle, but they were still reporting gas sales. He did not have a straight answer to that question. I compared the production reports with permit 4928 and found that they were identical. Jim claimed that he would plug the well, but he wanted to talk to the landowner first. I told Jim that I was not issuing a compliance notice at this time, but I know the well is idle and needs to be plugged.
1542129462 2018-11-13 Plug / Plug Back No Violations On the day of my inspection I found that LBJ Drilling had tubing ran to a depth of 3520 feet. Cemenco pumped a 300 foot bottom plug with 25 sacks of class A cement. Evan Dugan said that he was going to pull the tubing today and he wouldnt be back to rip the 4.5 inch casing until tomorrow morning.
1542228042 2018-11-14 Plug / Plug Back No Violations On the day of my inspection LBJ Drilling parted the 4.5 inch diameter production casing at a depth of 2518 feet. They pulled the casing from the well, and then they ran a pre-cast tapered cement plug in the well. LBJ Drilling set the tapered plug on top of the 4.5 inch casing at a depth of 2518 in order to establish a foundation for a clay plug at the top of the Big Lime. Jim Orr scheduled a clay delivery for Friday morning.
1542652746 2018-11-16 Plug / Plug Back No Violations Upon my arrival on location, I witnessed the rig setting up the mixing trough to set the Big Lime plug with clay. They mixed four tons (400 shovel scoops) of clay with water. They made a slurry with the clay inside the trough and let run down into the wellbore. Next, they prepared a Brush and Stone Bridge at the depth of 950 feet. This clay plug will cover the Berea zone. They mixed another 4 tons (400 scoops) of clay slurry in the trough and let it run down into the wellbore. They will try to pull the 8.625 inch diameter casing Monday morning.
1542743142 2018-11-20 Plug / Plug Back No Violations On the day of my inspection LBJ Drilling was rigging up to pull the 8.625 inch surface casing. Inspector Jeff Adams covered the plug job on Friday, November 16. He reported that LBJ washed 4 tons of clay on top of the pre-cast plug for a 200 foot plug at the top of the Big Lime. Then they set a brush and stone bridge at 950 feet and washed another 4 tons of clay in the well for a 200 foot plug across the Berea. LBJ made an attempt to pull the surface casing and it would not budge. They equipped the ripping tools to part the casing. When Evan Dugan ran in with the tools they sat down at a depth of 140 feet. He pulled the tools out and they were covered with clay. They shut down for the day to formulate a plan.
1542821279 2018-11-21 Plug / Plug Back No Violations On the day of my inspection I found that LBJ Drilling had the surface casing out of the well and laying on the ground. Jim Orr called me on 11/20/18 to report that they got through the clay that was bridged at 140 feet. He said that once they got through the clay the tools ran smoothly all the way the to the top of the Berea plug. They ripped the surface casing at a depth of 590 feet and pulled it. The crew mixed a clay slurry with fresh water and washed it down the well throughout the day. The well filled to surface multiple times and they stopped to allow it to settle. They had washed approximately 4 tons of clay down by the end of the day. I requested that they keep topping it off over the next few days and leave the well open until 11/26/18 to allow it to settle completely before capping the well.

1542986744 2018-11-23 Plug / Plug Back No Violations Upon my arrival on location, I witnessed the fluid level in the well was about 30 feet down from the surface. The rig crew shoveled another 75 scoops of clay into the wellbore. They are going to leave the well set over the weekend to see if it is done settling. They will check it again Monday morning.
1543247418 2018-11-26 Plug / Plug Back No Violations On the day of my inspection I found that the open casing was under water. I could not see the casing or the top of the clay. There was approximately 3 tons of clay left in the pile. I called Inspector Jeff Adams, who covered the job on Friday, Nov. 23. Jeff said that the fluid level was down 30 feet when he arrived and MFC washed another ton of clay in, filling it to surface again. I contacted MFC and they said that they would be out tomorrow to finish the job.
1543333940 2018-11-27 Plug / Plug Back No Violations On the day of my inspection MFC had a crew on site to finish the plug job. They excavated the soil from around the well, allowing the rain water to drain away and expose the casing. The well was still filled to the surface with clay. MFC cut the casing off 36 inches below grade and topped it with a steel plate that was inscribed with the permit number. MFC loaded the pulled surface casing on to a trailer and removed it from the location. They still need to fill in the cellar around the well casing and grade the site. They also need to remove the tank and lines. The landowner came down to express his concern that he wants the steel lines removed that run from the well, along the road side, up to the farm house. MFC has until May 27, 2019 to complete the restoration.
1543501408 2018-11-29 Plug / Plug Back No Violations On the day of my inspection I found that MFC had removed all of the pulled 8.625 inch casing from the site. The cellar had been filled in, the remaining clay pile was gone, and the site was rough graded. The 100 barrel tank and the old steel flow lines were still in place.
