0587974893 2018-10-17 Production Wells No Violations I noticed that the dike was holding water in one end when I was driving by this location. I was recently involved with a compliance notice on this well for brine in the dike, so I stopped to investigate. M&R Investments hauled soil out of here and rebuilt the dike to resolve the previous compliance notice. There were new deer tracks in the corner of the dike and scratches in the soil. I tested that standing water with a high range Quantab and found that the chloride level was less than 266 ppm(mg/L) or 0.047% NaCl. The chloride level in the dike water was within the EPA drinking water standards. No compliance notice was issued on this date.

1405703169 2018-06-07 Production Wells Violations Noted On the day of my inspection I noticed standing water and discoloration in the dike when I was driving east on CR 19. I stopped to investigate and found that the inside of the earthen dike was darkly stained, muddy and heavily tracked by deer. The dike was wet in the east end, and I also noticed the odor of brine water. There was one 100-barrel steel tank and one 30-barrel poly tank both set up on gravel. I did not find any signs that the steel tank was leaking. I did notice that the valve on the poly tank was wet and the gravel under the valve was stained. I tested the standing water with a high range Quantab, which maxed out at 6298 ppm(mg/L) chlorides. The chloride content of the water exceeded the Quantab scale. I also found that the water had been leeching through the east end of the earthen dike wall, and there was a brine scald that extended approximately 40 feet into the surrounding hay field. The brine scald in the hay field was approximately 10 feet across at its widest point. I took photographs and sent them to the field supervisor, Dale Smith. I also called Dale to let him know what I had found and to ask that he get a crew out here as soon as possible. All of the brine saturated soil in and outside of the dike will need to be excavated and hauled to an approved landfill. The dike wall where the brine has been leeching through will also need to be removed and rebuilt with clean soil. Dale and I discussed hauling in new topsoil to remediate the area outside of the dike once the affected soil had been removed. The leaking tank valve should be replaced or repaired.
1528896901 2018-06-13 Production Wells No Violations On the day of my inspection I found that Diversified had a crew on site working to remove the brine contaminated soil. They had already scraped the top layer of soil from the hay field and they were working on the dike. There was a water truck, dump truck and track-hoe on site. The poly tank was drained and laying on its side. I spoke with the crew leader who said they were removing the poly tank and decreasing the size of the dike. He said that they had a manifest at Kimbles landfill to dispose of the soil. He also said that they were bringing in new soil to put down in the hay field, and they would treat the whole area with lime and fertilizer.
1531160648 2018-07-09 Production Wells No Violations I visited this site with Trish Cartmell and Jason Blocker from the ERS team to follow up on the spill remediation. There was grass coming up on half of the remediated area and the other half was still bare. Trish and Josh said that they would like to revisit the sit in the fall to follow up on the vegetative growth. There was still a muddy area in the corner of the dike where rainwater was collecting and bringing chlorides to the surface. There were no signs of leaking from the tank and there were no signs of anything leaching out of the new dike. I called Chuck Woods with Diversified and explained what I found. He said that he would get back out there to remove more of the soil from inside of the dike.
1532094684 2018-07-19 Production Wells No Violations On the day of my inspection I met with Jason Blocker at the site to assess the extent of the brine impacted soil with his soil conductivity meter. A crew from Sutton Pump showed up while we were there. We discussed the areas that needed to be excavated, and Jason probed the soil through the excavation process. Sutton removed the affected soil and filled the area in with new stone.
1710769468 2012-01-25 Production Wells No Violations Inspection completed this date. The well is capable of production and equipped with pumping unit, 1-100 barrel storage tank and a separator.
1760386176 2018-06-20 Production Wells No Violations On the day of my inspection I found that the brine contaminated soil had been excavated and removed from inside and outside the dike. The poly tank had been removed and the area of the dike had been reduced. The dike walls had been rebuilt and the soil around the dike had been graded out with fresh topsoil. The whole area was covered with grass seed and straw. The compliance notice is resolved.
