0124985703 2015-05-26 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped with a pumping unit. The motor was intact, the valves were open and there was pressure on the gauges. The tank battery consisted of two 100 barrel tanks, one 30 barrel poly tank and an earthen dike. Identification was posted. There was an area of oily paraffin approximately 3 by 5 that needs to be cleaned up. This well is in production. The identification showed Hardesty Production (576) as the operator, and they reported production from 2009 to 2012. I called and left a message.

1296503738 2015-06-25 Production Wells No Violations On the day of my inspection I found that the well was still listed as Historic Owner. I also found that the paraffin had not been cleaned up. I contacted Mark Hardesty. He claimed that he has never received an ownership packet. He also claimed that the rain has prevented them from cleaning up the paraffin. He said that he would try to get out there as soon as possible. I emailed Kathryn Smith in bonding and permitting to send out a non-domestic well ownership packet.
1432735442 2015-05-27 Administrative Inspection No Violations I was contacted my Mark Hardesty of Hardesty Production, who claimed that they owned this well and were up to date on their production reports. I contacted Stephen Riley at DOGRM Central Office and he confirmed that production reports had been submitted for 2013 and 2014, but they were not applied to the well since it is listed as Historic Owner. I also contacted Kathryn Smith in bonding and permitting. She is sending a non-domestic well ownership packet to Mr. Hardesty to get the well on his bond. Mr. Hardesty also claimed that he would get out to clean up the paraffin.
1441376019 2015-09-04 Production Wells No Violations On the day of my inspection I found that the paraffin had been cleaned up. The well is still listed as Historic Owner. I called Mark Hardesty and left a message. I also sent another email to Kathy Smith in regards to sending out a non-domestic ownership packet to Hardesty Production.
1466288061 2017-07-19 Production Wells No Violations On the day of my inspection I found that the well was equipped with a natural gas powered pumping unit. The engine, drive belts and fuel line were intact. The flow lines were connected and the valves were open. The production equipment consisted of one 100 barrel steel tank and one 20 barrel poly tank in an earthen dike. The identification sign was for Hardesty Production. I called Mark Hardesty to ask what he was doing about getting the well in his name. He said that they sent the affidavit and form 7A in to Columbus two weeks ago. RBDMS is still showing this well as Historic Owner.
1516889441 2018-01-24 Production Wells No Violations On the day of my inspection I found that the well is no longer listed as a Historic Owner. The ownership has been transferred to Hardesty Production. The annual production reports have been brought up to date. There were no leaks around the pumping unit or production equipment. The identification posted on the 100 barrel steel tank was correct and legible.
1975307153 2016-01-19 Production Wells No Violations On the day of my inspection I found that the wellhead was equipped with a pumping unit. The combustion engine was intact with belts and gas line off of the casing. Duke Hardesty came out to the well during my inspection and started the well. He claimed that his family has owned the well for 46 years. The tank battery consisted of two 100 barrel tanks, one 30 barrel poly tank and an earthen dike. Identification was posted for Hardesty Production. There were no leaks or spills. Mr. Hardesty claimed that his brother Mark handled all of the paper work. I called Kathy Smith at DOGRM central office to ask about getting this well transferred. She said that I would need to send Mr. Hardesty a form 7A to get the well transferred since it is listed as Historic Owner.
