0361781581 2015-10-20 Production Wells No Violations On the day of my inspection I found that the exposed portion of the wellhead was equipped with a 5 to 2 swage, 2 valve, 2 nipple and 1 valve. There were no visible connections to the wellhead. I started to dig around the wellhead, but stopped when I detected a gas odor. There were two sections of disconnected 2 steel line laying on the ground next to the wellhead oriented toward a 100 barrel tank. There was also a 100 barrel tank next to Twp. Hwy 19. There were no visible fluid leaks at the wellhead or either tank.

1287499501 2010-10-19 Production Wells No Violations Complaint from landowner 10/15/2010 ( John Miller 330-695-9963 work home ) . Old abandon well on property, twp rd 19 off county rd 19. Inspected in 2003 no change in well status. See history for last inspection. Will need to search lease at recorders office.
1445438779 2015-10-21 Production Wells No Violations On the day of my inspection I dug around the wellhead and found that the 5 casing was on a casing hanger. I did smell a gas odor, but it did not register on the hand-held gas detector. I will turn this over to Idle & Orphan Well inspector Cindy VanDyke as a possible orphan well.
