0375613435 2016-07-12 Plug / Plug Back No Violations On the day of my inspection I found that the site had been rough graded. The 5.5 inch casing was still laying on the ground. There were still various sections of plastic flow line scattered around the site. There was still steel and plastic lines running along side the lease road. There were still two 100 barrel steel tanks in place.
0377518758 2017-09-08 Final Restoration No Violations On the day of my inspection I found that any of the vegetative growth established on the old lease road and well site had been disturbed by recent logging operations. The only oilfield related materials or debris that were left at the site were the sections of loose flow line left behind at the old tank battery location. I called and left a message with Travis Dugan about getting it removed. Dwayne Dugan called me later that day and said that he would personally go out the following day to remove all the remaining materials.

The production casing is open to the atmosphere and I was unable to determine if the well is actually producing oil. There is no identification (ID) posted at the well site The well owner must affix a valid and legible ID as soon as possible. This well must be put into production or plugged.
1113068191 2017-04-25 Final Restoration No Violations On the day of my inspection I found that there were still bare spots on the site from standing water. The access road was covered in leaves and it was also wet in certain areas. The pieces of flow line had not been removed yet. I called Dwayne Dugan and aske that he get the debris removed. I also suggested that he look into a landowner waiver for the establish vegetative cover requirement since the site is naturally wet.

1270616542 2016-06-23 Administrative Inspection No Violations Phil Moran called to report that the clay was still sitting firm at the surface. He reported that he cut the casing off, capped the well with a steel plate and buried it. The well has been plugged. This violation is resolved.
1466430304 2016-06-20 Plug / Plug Back No Violations On the day of my inspection I found that Phil Moran was rigged up to plug the well. The rods were out and Phil was ready to pull the tubing. I talked to Dwayne Dugan about the plug job. He said that he intended to plug all three wells on the Mossholder lease before the permits expire on 7/2/16. The clay showed up while Phil was pulling the tubing. They got 22 tons from Logan Clay. Phil decided to shut down after pulling the tubing, with intentions to start running clay the following morning.
1466526876 2016-06-21 Plug / Plug Back No Violations On the day of my inspection Phil Moran washed 5 tons of clay down the 5.5 casing, then he rigged up to rip the 5.5 casing. He could not get any stretch when he tried to pull on the casing. He could not get a telegraph until 955. The 10 conductor was open with the 5.5 on a casing hanger. There were no other visible casing strings in the hole. The well was drilled in 1937. The 8 and 6 casing strings were likely removed during the WWII era and never recorded as being pulled. The 10 casing has likely been open for decades, allowing water, soil and debris to wash down the annulus around the 5.5 casing. Phil set a brush and stone bridge in the 5.5 casing at 1000 and then he ripped the 5.5 casing at 955. They pulled three joints and shut down for the day.
1466602763 2016-06-22 Plug / Plug Back No Violations On the day of my inspection Phil Moran finished pulling 955 of 5.5 casing from the well. The casing was dragging hard and there was a coating of fire clay residue on the outside. It is likely that fire clay was washed down the annulus after the 8 and 6 casing strings were pulled. They washed approximately 15 tons of clay down the well bore, filling it to surface. The well filled to surface multiple times. The well was filled one final time and left to settle overnight.
1481643214 2016-12-13 Final Restoration No Violations On the day of my inspection I found that the well site had been seeded and was 80% covered in short grass. The well site was in a wet bottom and there were still some bare spots where water had pooled on the surface. The access road was covered in so many leaves that it was difficult to determine bare spots from grass coverage. There were still multiple section of plastic and steel lines in the edge of the tree line from the tank battery site back to the well site.
1503929407 2017-08-28 Final Restoration No Violations On the day of my inspection I found that the property was being logged. The old lease road and well site were worn and bare from logging vehicle traffic. There were still some sections of flow line where the tank used be located. I called Travis Dugan to ask that he get the remaining materials and debris out of there. I told him that once the flow lines are removed I will pass the restoration., since the logging company had tore up the road and old well site.
