0592027553 2017-09-26 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging activities on well #6-2774. Operator Bob Foltz reported bailing the well dry on 9/25/2017 and that the fluid level had recovered to 500 feet below surface. Mr. Foltz continued to bail the well dry prior to the arrival of 9 sack grout. Total measured depth was 821 feet. R.W. Sidley delivered 1.25 yards of 9 sack grout. The well was dead. After pouring the grout into the well, the top of grout was measured at 558 feet below surface. Mr. Foltz continued by pulling the 4.875 casing. A total of 526 of casing was removed from the well. The well head was excavated and no conductor was present. The earthen pit continued to hold fluid. The location remained stable. I will schedule to witness the placement of the top plug. No violations were observed during this inspection.
1215909075 2017-09-21 Plug / Plug Back No Violations The purpose of my inspection was to conduct a status check on plugging operation on well #6-2774. Operator Bob Foltz was setting up equipment to plug the well. The location was stable. No pit had been dug yet. No violations were observed during this inspection.
1506450012 2017-09-25 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging activities on well #6-2774. Operator Bob Foltz was on location removing rods and tubing from the well. The fluid level in the well was measured at 218 feet below surface. Mr. Foltz bailed production fluid from the well to a lined pit. The pit held fluid and free board was maintained. I witnessed the fluid level bailed down to 530 feet below surface. I will schedule to be on location to witness the setting of the bottom grout plug. No violations were observed during this inspection.
1753069851 2017-09-27 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #6-2774. Owner/ Operator Bob Foltz reported the top of cement on the first isolation plug was 536 below surface. The static water level in the well was measured at 150 below surface. Two inch tubing was placed in the well to 511 below surface as a tremie line. R.W. Sidley delivered 5.0 yards of 9 sack grout. The grout was poured into the tubing annulus and displaced the fluid into an earthen pit. The pit was holding fluid and freeboard was maintained. Grout was not brought to surface. Mr, Foltz stated the well would be topped off when plugging well #2-0292. No violations were observed during this inspection.
1932035965 2018-06-12 Final Restoration No Violations The purpose of my inspection was to determine the status of final restoration on well #6-2774. At the time of my inspection, the working pit was backfilled and the location graded to local contour. The location was within a woodland. Vegetation was established. No signs of erosion or sedimentation were observed. All equipment and debris utilized in plug and abandonment was removed. The access road was reclaimed. The location has passed the requirements for final restoration.
