0005254554 2017-05-03 Production Wells Violations Noted The purpose of my inspection was to conduct a status check on well #6--DOGRM. The surface casing head was above grade and equipped with tubing and rods. No audible or visual leaks were identified. Some residual staining was present around the wellhead. The staining was not fresh or live. I confirmed the location of the tank battery with owner Shawn Watson. The tank battery consisted of three (3) stock tanks. Per a telephone conversation Mr. Watson, two vertical tanks were utilized for crude oil storage and a horizontal tank was utilized for water. He also stated that the tanks were empty of fluid except for possibly some residual water on bottom. The tanks remained in good repair. I did not observe any holes or leaks. Load out valves were closed and plugged. Hatch lids were closed. During my conversation with Mr. Watson I communicated to him the lack of identification at the well and tank battery. Correct and legible identification shall be posted at the well head and tank battery within 14 days. Identification at the tank battery shall indicate each well name and permit number produced into the battery. The well was incapable of production. Under 1509.62 (A)(1) the owner of a well that has no reported production for two consecutive reporting periods as reported in accordance with section 1509.11 of the Revised Code shall plug the well in accordance with section 1509.12. 0878413979 2006-03-13 Production Wells No Violations Inspected this date to verify the permit number with the GPS coordinates. The tank battery is shared with 14 other Berea wells and consists of 1-100 bbl tank, 1-75 bbl tank and 1-25 bbl tank. There is no identification posted at the wellhead or tank battery, the well is idle and there is no bond.

-4 gallons, has sprayed onto the ground, the surrounding vegetation, and all the over the well equipment. The tank battery consists of two 100 BBL production tanks and a horizontal 100 BBL production tank. No identification was posted. Well not capable of production. I called Shawn Watson at 11:53AM and told him about the mess and that he needed to place a plug as previously requested of him in the well. He said he would be out this afternoon to place a plug and clean up the mess. I received a follow-up call from Shawn Watson at 4:06 PM saying he had been out to the well and placed a plug in it. He said he would have it cleaned up by Tuesday 3/31/2015 because he had treatment.

1639533209 2019-01-17 Production Wells No Violations The purpose of my inspection was to follow-up on compliance notice #-5254554. RBDMS indicated the Owner reported zero production in 2017. The location remained stable. No identification was present. No audible or visual leaks were observed. The tank battery consisted of three (3) stock tanks. The tanks remained in good repair. I did not observe any holes or leaks. Load out valves were closed and plugged. Hatch lids were closed. The tanks were empty. I spoke with Shawn Watson on 01/25/2019 regarding the identification and leak. Mr. Watson reported posting identification at the well and would revisit the location to replace it. He continued that he was working to finance plug and abandonment on the well.
