The 9.625 and 5.5 valves were open. The 9.625 annular gauge indicated 0 psi. The 5.5 annular gauge indicated 100 psi. No leaks were observed. Production equipment on the pad consisted of a well specific sand knock out and GPU and a horizontal dehydration unit. Common equipment consisted of one (1) hydrate inhibitor tank, one (1) thermal generator, and one (1) meter run and one (1) VRU. The tank battery consisted of three (3) 400 bbl produced water tanks. The tank were inside an impervious lined steel containment. Valves were closed and plugged. The containment was holding fresh water. No violations were observed during this inspection. |

1658684719 | 2017-02-01 | Production Wells | No Violations | The purpose for my inspection was to conduct a status check on well #2-1879-00-00. The well is currently not in production. The well has been drilled to total depth but not stimulated at the time of my inspection. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. At the time of my inspection, the local area had received a coating of snow that partially covered the location. The lease road was also utilized for farming and remained drivable. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile was stable with well-established vegetation. An earthen dike berm is erected around the well pad. A chain link fence with barbed wire top is constructed around the pad to prevent unauthorized entry. An Emergency Shut Down (ESD) button was adjacent to the main entrance gate. The main entry gate and escape doors positioned around the perimeter were found to be closed and locked. A French drain was in place on the pad. A standpipe drain on the North side of the pad indicated shut. Four wells are currently present on the pad, only two are in production. The well heads are protected by a perimeter of concrete jersey barriers. No identification is present at the #10H well head. The cellar is open and contains water near freeboard. Steel grating was in place to prevent falls into the cellar. The mousehole was cemented. No pressure gauges were present on the well head. The gate valves on the well head assembly were chained and locked. No audible or visual leaks were detected. Production equipment on the pad consisted of two (2) vertical sand knockouts, two (2) gas processing units (GPU), one (1) hydrate inhibitor tank within containment, one (1) meter run, two (2) 400 bbl production tanks and one (1) vapor recovery unit.
