0115540059 2017-11-16 Production Wells No Violations The purpose of my inspection was to witness the setting of downhole retrievable bridge plugs in well #2-1877. Well #2-1877 was being temporarily shut-in to faceplate the drilling of two additional wells on the pad. Prior to my inspection the well tubing had been stripped out. At the time of inspection, the well head production assembly was removed. The production equipment on the location was disconnected and depressurized. KLX Wireline was contracted to set the plugs. After running a gauge ring to the designed setting depth, KLX inserted the retrievable bridge plug into the well under pressure. After setting the plug, the well pressure was released at surface into a small tank containing a gas buster. The annular pressure gauges on the surface and intermediate casing string each indicated 0 psi. After conducting the negative test, a second plug was to be set approximately 100 feet above the initial plug. No violations were observed during this inspection.
0191222967 2018-01-04 Production Wells No Violations The purpose of my inspection was to document the status of well #2-1877. Drilling had concluded on the pad. Correct identification was present. Well #2-1877 remained night capped and covered with a cage. Annular pressure gauges on the 9.625 and 5.5 casing strings both indicated 0 psi. No leaks were observed. Production equipment on the pad remained isolated. No violations were observed during this inspection.

-lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there two site drains installed at the northeast and southwest corners of the pad. Outside the fence, there is an earthen berm, with well-established ground cover. The soil stockpile is well stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. Preliminary restoration is complete.

1180851805 2018-05-10 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1877. The well is one of four production wells on the pad. Correct identification was present at the well entrance. The lease road and pad remained in good repair. I met production consultant Bryan Waid at the pad entrance. Mr. Waid reported the newly completed wells were being monitored due to sanding issues at surface. At the time of my inspection the 9.625 annular valve was open and the installed gauge indicated 0 psi. The 5.5 annular valve was open and the installed gauge indicated 200 psi. The cellar was backfilled with gravel. No audible or visual leaks were observed. While on location a Stallion vacuum truck arrived on location and was loading production fluid from the tank battery. I did not observe any leaks, spills or malodors. No violations were observed during this inspection.

At the time of my inspection well #2-1877 was night capped due to drilling activities on the pad. A steel cage was placed over the well head. The 9.625 and 5.5 annular valves were open and the pressure gauges both indicated 0 psi. No violations were observed during this inspection.
1495713403 2017-05-16 Production Wells No Violations The purpose of my inspection was to witness the loading of production water as a follow up to Complaint #5083. I spoke with Complainant Leonard Miller on 4/4/2017 to inquire if he had experienced any chemical odors similar to the one that had initiated the initial complaint. Mr. Miller stated that he had detected periodic odors since the initial complaint. From his experience as a water truck driver, he believed the odor correlated to the hauling of production fluid from the pad. Also, Mr. Miller stated the odor was noticeable when the wind direction was from the Northwest, in the direction of the pad. I communicated to Mr. Miller that I had conducted a status check inspection on 2/1/2017 and had not detected any chemical odors. Further, I told Mr. Miller that I would schedule a time with the Owner to be present on location while production fluid was being loaded. I arrived on location prior to the load out and walked the pad and did not detect any chemical odors relating to well operations, nor did I observe any evidence of leaks or spills. Weather conditions were sunny and warm with a slight breeze from the Southwest. Chesapeake lease operator Michael Hostetler arrived on location followed by a 120 bbl vacuum truck operated by Stallion Oilfield Services. Three production tanks are on the pad. Two are designated as holding produced water, one is designated as holding condensate. Per Mr. Hostetler, the two wells on the pad produce little to no condensate. A vapor recovery unit was on location and functioning. I remained on location during the loading process and did not detect any airborne chemical odors. I did not witness any fluid spills or leaks. I contacted Mr. Miller and discussed my observations and communicated to him that I would be closing the current complaint. I added that if he or someone experienced similar conditions to contact myself or ODNR-DOGRM and the complaint would be reopened and investigated.
1523386498 2018-04-05 Production Wells No Violations The purpose of my inspection was to conduct a status check of well #2-1877. I met Chesapeake representative and location supervisor Bryan Waid on location. Flowback operations for well 1895 and 1896 were occurring at the time of inspection. Correct identification was present at the well head. The intermediate and production casing annular gauges remained labeled and plumbed above grade. Valves were in the open position. Each gauge indicated 0 psi. No leaks were observed. I did not inspect the separator due to the active work in the area. The tank battery remained in good repair. No violations were observed during this inspection.
1583839262 2016-12-06 Production Wells No Violations The purpose for my inspection was to follow up on Complaint # 5175. The complainant stated that on the evening of 10.27.2015 she heard a noise in the kitchen and observed dishes inside a cabinet shaking. I called and spoke with the Complainant to inquire if there hade been any observable damage or additional instances of shaking since filing the complaint. The complainant re-stated that she felt well # 2-1877 was the source of shaking, but that she had not observed any damage. Further, the complainant stated that she had not experienced any additional instances of shaking. I communicated to her to call ODNR-DOGRM if the situation occurred again and that I would be closing the open complaint. At this time, no further action is necessary.
2087607679 2016-03-31 Production Wells No Violations I was on location for a status check. My inspection found the access road gate locked and the

access road is in good repair. An identification sign at the access road entrance, well, and tank battery. The pad is fenced and all man doors are locked. The #6H well is in production. The well is equipped with a christmas tree valve assembly. There is a steel dike around the tank battery. The equipment on the pad consists of the following: 2 - Storage tanks 400 bbl 1 - Vapor recovery unit (VRU) 2 - Gas processing units (GPU) 2 - Sand separators
