0430781615 2014-06-17 Preliminary Restoration No Violations I stopped by the site to check on the status of well completion and restoration. The well has been hydraulically fractured. There is three 400 barrel tanks and three GPU production units. The well is equipped with a Christmas tree. Values are locked out. Tanks or the wellhead do note have ID signs posted. I walked the entire site, and found the location to be in good condition. There is various values and pipe on location that needs to be removed. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized until well is in production and ID sigs are posted and location clear of all material laying on location.

-based mud used to drill and condition the hole the cement slurry was mudscaled to verify the target density, and . Nabors pumped 572.5 bbls of cement slurry at 14.4 ppg. The plug was dropped and displaced with 304 bbls of water as measured by pump truck tanks. The plug bumped the float collar at 3:43pm on 12/12/2013. The plug landing pressure was 2510 psi, and it was raised to 2965 psi, and held for 5 minutes. The float held when the pressure was released. 3.0 bbls of water bled back.

The entry off SR14 was done and the access road was built and covered with limestone aggregate. The drilling pad was largely built, and covered with limestone aggregate. A construction crew was still finishing the drainage, erosion and sedimentation controls surrounding the pad and access road.

2011348866 2016-08-23 Production Wells No Violations The purpose of my inspection was to determine the status of preliminary restoration and conduct a status check of the Firestone Homestead Pad B with Regional Emergency Operations Manager Terry Whitehair, and Emergency Operations Coordinator Fred Romeo. Atlas Lease Operator Nelson Case provided entry to the pad and served as a guide for the inspection. Access to the pad is over a lease road that is lined with crushed gravel and remains in good repair. A sign board posted near the entrance identified the operator, well names and permit numbers, and emergency contact information. A chain link fence topped with barbed wire is present around the entire pad. An emergency shut down button is present at the entrance and at all four corner escape doors. I walked the perimeter of the pad and found the exterior perimeter stable with well- established vegetation. The topsoil stockpile is stabilized with well-established vegetation. The surface of the pad was composed of crushed gravel that remained in good repair. Equipment and material not used in the production process have been removed. The Firestone Homestead Pad B currently contains three (3) wells. Firestone Homestead Pad B #4H (API # 2-1830-00-00) is identified with a steel dog tag that contains correct and legible identification. After stimulation, the well was equipped with a plunger lift valve assembly. The casing pressure gauge indicated 920 psi. The tubing pressure gauge indicated 796 psi. The cellar had plastic pipe installed to facilitate draining and was backfilled with gravel. The mousehole was cemented. No audible or visual leaks were detected. Production equipment on the pad consisted of three (3) gas production units (GPU), having one unit for each respective well, and three (3) 400 bbl production tanks. The GPU contained correct, legible and conspicuous identification and sat adjacent to the units for the other two wells on the pad. All three units were placed on top of containment that rema
