0000686885 2017-07-13 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #1717. Correct signage remained posted at the entrance. The access road remained in good repair. No evidence of accelerated erosion or sedimentation was present along the road. The pad was enclosed by a locked chain-link fence perimeter. The fence remained in good repair with no signs of vandalism. An earthen berm perimeter was present outside of the fencing. I walked the perimeter of the pad and found no evidence of erosion or sedimentation. The topsoil stockpile remained stable. The pad was composed of crushed gravel that remained in good repair. A jersey barrier perimeter was present around the well head. Correct signage was present on the well head. Annular pressure gauges were identified. The 13.375 annular gauge identified 0 psi. The 9.625 annular gauge identified 0 psi. The 5.5 production casing and tubing gauges identified 0 psi. No violations were observed during this inspection.
0089975473 2012-11-30 Drill / Deepen / Reopen No Violations

-Tex Pumping was the cementing contractor. There was good fluid circulation throughout the cementing operation. The cement job was done in stages. Following a spacer designed to recover the oil-based mud used to drill the hole and condition the hole, a lead scavenger cement followed by a tail cement were pumped. The lead scavenger cement was mudscaled at 13.0 pounds per gallon (ppg), and O-Tex pumped 226 bbls of lead cement at 13.0 ppg. O-Tex then pumped 480 bbls of tail cement at 15.3 ppg. The plug was dropped, and displaced with 320 bbls of water. No cement returned to surface, but approximately 40 bbls of spacer returned to surface. The plug bumped the float collar at 9:50am on 11/30/2012. Pressure held. The float held when the pressure was released.

0275487865 2016-06-17 Production Wells No Violations My purpose for inspection was to conduct a second year temporary inactive well status inspection.

There is one well on the pad. The sign board posted at the lease entrance displayed signs with the well name, address, permit number and emergency contact information. The lease road was graveled and in good repair with a locked gate at Cream Ridge Road. Erosion and sedimentation controls were in place and ground cover was well established along the road. I walked the entire location and found the well pad constructed with an earthen berm perimeter in good repair, being stabilized with well-established vegetation. Erosion and sedimentation controls are in place and functioning. The well pad was found to be enclosed by a chain link fence with barbed wire at the top. The primary entrance and escape doors were all found to be locked. The surface of the well pad consists of gravel and is in good repair. The well pad was not constructed with a site drain and several areas on the pad contained ponding water. Soil stockpile is stabilized with well-established vegetation. The Reed 8H was constructed with 145 of 20 conductor, 672 of 13 3/8 surface casing, 1619 of 9 5/8 intermediate casing and 14, 559 of 5 1/2 production casing. Drilling was completed 11/28/2012 and stimulated 2/16/2013. The well head was found to be protected by a perimeter of steel cattle gates that were further surrounded by concrete Jersey barriers. Proper signage was displayed. A cellar grate was in place to prevent falls. The cellar contained water with approximately 1 foot of freeboard. The production casing gauges indicated 0 psi. I observed an annular pressure gauge that indicated 80 psi, however I could not identify what casing string the gauge belonged to due to the water level in the cellar. No sheen or leaking gas was observed on the water in the cellar. The mousehole was cemented. Gate valves are chained. No other production equipment is on the pad. I recommend that th

He indicated that it would be take care of on 09/19/2012. I stopped at the site to check on the status. I found the cellar installed, and the conductor casing topped off with cement.

The Owner raised and identified the 13.375 surface casing and 9.625 intermediate casing. The surface casing gauge indicated 0 psi. The intermediate casing gauge indicated 0 psi. No further action is necessary.

0795961952 2014-05-20 Production Wells No Violations I stopped by the site to check on the status of well completion and restoration. The well has been hydraulically fractured, but no production equipment is on site. I walked the entire site, and found the location to be in good condition. All erosion and sedimentation controls, and drainage controls appear to be functioning. Ground cover is well established along the access road and around the drilling pad. No areas of erosion are present. Preliminary restoration approval will not be finalized pending installation of production equipment. All valves are chained and locked.

Due to the water level in the cellar, I was unable to determine the pressure indicated on the submerged gauges. No sheen or leaking gas was observed on the water in the cellar. The mousehole was cemented. Gate valves are chained and locked. No other production equipment is on the pad. I request that the water level in the cellar be drawn down to a level where annular pressures can be determined and monitored while the well is in inactive status.

1548325879 2013-07-23 Preliminary Restoration No Violations I reviewed the site to assess the completion of preliminary restoration. There is a sign board at the access road entry featuring the well name, permit number and emergency contact information. The access road has a padlocked gate near Cream Ridge Road. The access road is in good repair. Drainage and erosion controls were in place and appear to be functioning. Ground cover was well established along the access road. I found the drilling/production pad to be enclosed entirely with a chain link fence with barbed wire at the top, and with a padlocked gate. The pad has a crushed limestone surface, and is in good repair. The pad does have some ponded water along the perimeter. The fence is in good repair. Outside the fence, there is an earthen berm. I walked the exterior perimeter and noted that the berm is in good repair and ground cover is well established. Most of the area of disturbance has good ground cover. The soil stockpile is well stabilized with vegetation. It appears that a site drain with shutoff valve has been recently installed on the west side of the pad. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The well has been fracked, and has a Christmas tree valve assembly. There are bull fences and concrete barriers surrounding the wellhead and cellar to prevent vehicle impacts. There are grates over the cellar to prevent falls. There is no production equipment at the site. The cellar has water to within 2 to 3 feet of the top. Preliminary restoration approval will not be finalized pending installation of production equipment and re-inspection.

The road and well pad remained in good repair. The perimeter security fence remained in good repair and locked. The perimeter berm and slopes remained stable with well-established vegetation. The French drain stand pipes remained open. Correct identification was present at the well head. Jersey barriers were present to prevent a collision. Grates remained over the cellar to prevent falls. The cellar contained freshwater. No leaks were observed. Gauges to measure annular casing pressure were present and labeled. The 9.625 annulus indicated 0 psi. The 5.5 annulus indicated 0 psi. The 5.5 production casing wing valve gauge indicated 0 psi. The production tree cap gauge indicated 0 psi. All valves were chained, closed and locked. No production equipment was on location. No violations were observed during this inspection.
