--lined and sealed secondary containment. There are concrete barriers protecting the containment from vehicles. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Along the interior of the chain link fence there are two site drains installed at the north end of the pad. Outside the fence, there is an earthen berm, with well-established ground cover. 0891136754 2017-01-26 Production Wells No Violations The purpose for my inspection was to conduct a follow up status check on well #2-1702. The inspection was conducted with ODNR-DOGRM Northeast Regional Manager Thomas Hill and Senior Health Physicist Robert Leidy. RBDMS indicates the operator is current with reporting production. The lease entrance is marked with a sign board containing signs reporting required well identification and emergency response information. The lease road was composed of crushed gravel. The road was stable and drivable. No signs of erosion were observed adjacent to the road traveling back to the pad. The soil stockpile remained stable with well-
1439436615 2017-08-21 Production Wells No Violations The purpose of my inspection was to conduct a status check on well#2-1702. The lease road and well pad remained in good repair. The topsoil stockpile remained stable. A locked chain link fence was constructed around the facility. No signs of vandalism were present. A stable earthen berm was present outside of the fenced perimeter. The pad was shared with 2-#2-1697 and #2-1701 . Correct identification was present. The well cellar contained fresh water and was covered by a steel grate. No leaks were detected. The 9.625 annular pressure gauge indicated 3.3 psi. The 5.5 annular pressure gauge indicated 425.4 psi. The 5.5 production casing was equipped with a plunger lift assembly. Production equipment on the pad consisted of (1) sand knockout and GPU for #2-1702, one (1) hydrate inhibitor tank, one (1) meter run, one (1) one compressor, three (3) 400 bbl produced water and three (3) 400 bbl condensate tanks inside steel containment and one (1) vapor recovery unit. The production tank containment was lined with an impervious liner and was holding fresh water. No violations were observed during this inspection.

2060384333 2018-09-14 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1702. RBDMS indicated reported production was current. Correct identification was posted at the entrance. The access road remained stable and in good repair. The pad security fence remained locked and in good repair. The freshwater impoundment to the North of the location was holding freshwater with freeboard. Correct identification was at the well head. The intermediate and production casing annular gauges were plumbed above grade and labeled. The intermediate annular gauge indicated 2.6 psi. The production annular gauge indicated 206 psi. The well head was equipped with a plunger lift assembly. The casing gauge indicated 60 psi, the tubing gauge indicated 268 psi. Production valves were in the open position. No leaks were observed. Production equipment on location consisted of three (3) sand knock out and gas processing units, one (1) production meter run, one (1) compressor building, six (6) 400 bbl production tanks and one (1) vapor recovery unit. No issues were observed with the production equipment. The compressor was removed. No violations were observed during this inspection.
