0235203292 2016-10-31 Production Wells No Violations The purpose for my inspection was to follow up on action requested concerning illegible identification and gas leaking from the domestic supply connection. The identification posted on the production tank is correct and legible. No audible or visual leaks are present at the well head. No further action is necessary.
0305555733 2008-09-23 Drill / Deepen / Reopen No Violations Inspection revealed that G & H Drilling has TDed the well and they are getting ready to log the well Appalacian Well Services arrived on site at 3:25pm to log the well. No violations at this time. The rig crew advised that they are suppose to move to the Bock lease next ( permit # 1662).
0644568099 2008-09-18 Drill / Deepen / Reopen No Violations On site again to witness the surface job. G & H Drilling finished coming out of the hole with their drill string and started into the hole with the surface casing . They got approximately 3 1/2 joints of casing into the wellbore and it stopped going downhole. They worked the casing up and down and turned it with the power tongs and it wouldnt go down the hole. They decided to trip the surface casing back out of the hole and go back to bottom again with the bit and drill string and circulate the hole for a while with heavy drilling mud. I advised that I was leaving and for them to contact when they were ready to run the surface casing again. No violations at this time.

1287213523 2008-09-16 Drill / Deepen / Reopen No Violations Inspection revealed that G & H Drilling is at 114 . They advised that they should be ready to set surface tomorrow night or early Thursday morning. No violations at this time.
1848183786 2008-09-18 Drill / Deepen / Reopen No Violations On site to witness G & H Drilling run surface casing . The rig got seven and a half joints of casing into the wellbore and then it wouldnt go any further. The rig worked the casing trying to get it to go down the hole. The casing wouldnt go down the hole so they decided to pull the casing and go back into the hole with the drilling bit and drill string . I advised them to let me know when they got back on bottom and were ready to come out of the hole again. The Toolpusher ( Jeff Craig ) advised that they probably wouldnt be ready to run casing again until later this evening.
2065591804 2016-10-20 Production Wells No Violations The purpose of my inspection was to conduct a status check on well # 2-1663. RBDMS indicates the operator is current with reporting production. The lease road is also utilized for agriculture and remains stable and in good repair. The well location is stable with well-established vegetation. No identification is posted at the well head. The tank battery is within view and contains an identification sign. The 8.625 surface casing head is above grade and contains a plug in one side port and a valve in the closed position in the other port. The 4.5 production casing is equipped with a plunger lift assembly. Production valves are in the open position. A domestic supply connection is present. Gas was observed leaking from threaded fittings on the domestic supply drip. The tank battery is enclosed by an earthen dike that is stable and sufficient for containment. Production equipment within the dike consists of one (1) vertical separator and one (1) 100 bbl production tank. Minimum spacing requirements are met. The separator remains in good repair. No audible or visual leaks were detected. A current production chart was in the meter house. The production tank remained in good repair. No leaks were detected. An identification sign posted on the tank stairs was illegible. The load line valve was closed and contained a plug. The brine drain valve was closed and contained a plug. The hatch lid was closed. While on location I called and spoke with Vanguard field supervisor Russell Miller concerning the illegible identification and gas leaks on the domestic supply. Mr. Miller indicated the sign would be corrected today and he would also investigate the responsible party for the domestic connection at the well head and either make necessary repairs or contact the supply owner to make necessary repairs.
