0052324739 | 2016-08-15 | Production Wells | No Violations | The purpose of my inspection was to conduct a general status check on the Jarman #1. Well # 2--tube wellhead assembly. A depression had formed adjacent to the wellhead and contained water from a rain event earlier in the day. I tested the water with a low range Quantab and realized the chloride level of the water exceeded the maximum range. I followed up that test with a high range Quantab that registered at a 7, or approximately 2521 ppm of chloride. The total surface area of impact is approximately 120 square feet. No audible leaks were observed. The tank battery was enclosed by an earthen dike stabilized with well-established vegetation. Within the dike, the production equipment consisted of a horizontal separator and a 100 bbl production tank. The separator remained in good repair. The inlet side of the separator had production shut-in. The production tank remained in good repair. No leaks were observed. The load line valve was shut and plugged. 0253477370 | 2016-08-17 | Administrative Inspection | No Violations | On 8/17/2016, I placed a phone call to Duane Clark at Petrox concerning my field inspection of the Jarman # 1, permit # 2-1467 on 8/15/2106. I reported to Duane my observation of oil and brine leaking to the ground underneath the well head. He informed me that he would instruct his contract well tender to contact me regarding any necessary repair and remediation. On the afternoon of 8/17/2016, I received a phone call from Jeremy Kolar who is contracted to tend the wells for Petrox. I communicated to Mr. Kolar the issues I observed and instructed him to excavate the area adjacent to well head to evaulate the extent of contamination and condition of well equipment below grade. Once the work was completed, Mr. Kolar agreed to call me so that I may schedule a follow-up inspection to determine future action. |
1264077581 | 2016-08-24 | Production Wells | No Violations | On 8/22/2016 I received a phone call from Jeremy Kolar of Kolar Operating, LLC. indicating that he had hand dug an area around the well head of the Jarman #1 so that I may evaulate the extent of contamination and condition of the well head. On 8/24/2016 I returned to the location to inspect the conditions on site. Upon arrival I discovered a small area around the well head had been excavated to expose the 8.625 casing slip head. The 4.5 annular valve was open. I did not observe any audible or visual leaks from the production casing annulus. My determintation is the source of the leak is within the bottom of the U-tube well head assembly. I will contact Jeremy Kolar to discuss excavating the area to remove the affected soil along with repairing/ replacing the hardware causing the leak. Once complete, I will plan to reinspect the area prior to having the location restored and stabilized. |
