0622333790 2017-04-19 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-1404. A review of RBDMS indicates the owner is current with reporting production. Access to the well was by foot through a reclaimed strip mine. The well location was stable with well-established vegetation. No identification was posted at the well head. The 8.625 well head was above grade. The annular side ports were open to atmosphere. The 4.5 production casing was equipped with a plunger lift assembly. Production valves were in the open position. No audible or visual leaks were identified. The tank battery was location was at the edge of a wood line and was stable. An earthen dike that was stable and free of fluid was constructed around the production equipment. Equipment within the dike consisted of one (1) horizontal separator and one (1) 140 bbl production tank. The separator was in good repair. No audible or visual leaks were identified. A current 31 day chart was in the meter house. Correct and legible identification was on the tank body. The tank body exhibited extensive exterior corrosion. No holes or leaks were identified. The load line and drain line valves were closed and plugged. The tank was resting on two timbers. The hatch lid was closed. I will contact contract lease operator Jeremy Kolar to post identification at the well head.
