RBDMS shows the operator has reported production within the last two years. The lease road to the well and tank battery is stable and in good repair. The well site is stable with well--tube assembly causing contamination to the ground around the well head. The area of disturbance is approximately four (4) feet in diameter. A domestic supply connection was also present. The tank battery was enclosed by an earthen dike that was insufficient in capacity to contain a production release in the event of equipment failure. The tank battery is upgrade and within close proximity to the headwaters of the Middle Branch Sandy Creek. The production equipment within the dike consisted of one (1) horizontal separator, one (1) 210 bbl production tank and one (1) buried brine vault. Minimum spacing requirements are met. The separator was in good repair. No audible or visual leaks were detected. A current 7 day chart was in the meter house. The production tank was leaking production from several small holes in the tank body. Correct, legible and conspicuous identification was present. The load line valve was closed and plugged. The brine drain valve was in the closed position and plumbed to the vault. The brine tank remained in good repair and was holding fluid. No evidence of overfilling existed. I will contact Quality Oil & Gas Corp. to have the well head leak repaired, clean up the contaminated area, and make repairs to the production tank body leaks. |

Contract lease operator Dave Wittick was on location during the inspection. During my inspection on 8/24/2016, I observed production fluids leaking from well head plumbing connections, ground contamination from the well head leaks, production fluid leaking from holes in the production tank body and an insufficient dike. During the follow-up inspection, I observed the connections that were previously in poor repair to have been repaired. The contaminated surface area below the well head was cleaned up. The upper hammer union connection on the U-tube assembly was observed dripping production fluid (brine) to the ground surface. Mr. Wittick acknowledged the leak and informed me that he would schedule a plumber out to repair the leak within the week. Concerning the tank battery, Mr. Wittick informed me that the production tank was no longer serviceable and was scheduled for replacement within the week. Following replacement, the earthen dike would be repaired and made sufficient for containment. |
