0740361693 2017-07-25 Production Wells No Violations The purpose of my inspection was to document well head leak repairs to well #2-0898. I received a voicemail from Dorfman production manager Paul Shaw on 7.25.2017 stating that a repair crew would be on location to excavate the well head area and make necessary repairs to stop the well head leak that day. At the time of my inspection, the 8.625 well head was exposed. The 4.5 annular riser was replaced. While on location I spoke with Mr. Shaw who stated the source of the leak was the threads of the 4.5 annular nipple. New riser hardware was installed and plumbed above grade with a closed and plugged valve. No audible or visual leaks were detected. The location remained stable. No further action is necessary.
1185130284 2015-06-30 Production Wells No Violations I was on site to conduct a status check. The tank battery access is separate from the wellhead access and is located at 40.81529, -80.99214 +/- 8 ft. Identification was posted at the tank battery. The tank battery is shared between Stoffer #6 (20890) and the Stoffer #7 (20899). The tank battery consists of a 210 bbl production tank and a horizontal separator. There is a meterbox with a current 7 day chart showing production. A dike is present and in good repair. The wellhead is equipped with an electric pumpjack and a domestic supply line. Gas is bubbling up around the wellhead. I contacted pumper Jeremy Kohler at 12:49 PM on 6/30/15 and he said he would be out to dig up the wellhead and find the leak and fix it and give me a call when it was complete. Access road and site are stable. No other violations were observed.
2136446840 2017-07-03 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0898. The lease road, well and tank battery were stable. The 4.5 production casing was equipped with an electric powered pump jack. Correct and legible identification was present. I observed gas leaking from the ground beneath the 4.5 tubing head. The tank battery was shared with #2-899 and consisted of one (1) horizontal separator and one (1) 210 bbl production tank. The separator was in good repair and contained a current 31 day production chart in the meter house. An earthen dike that was stable and holding freshwater was constructed around the production tank. Correct and legible identification was present on the tank body. Valves were closed and plugged. No leaks were observed. While on location I called and spoke with Dorfman Production Manager Paul Shaw to make him aware of the well head leak. Mr. Shaw stated it was likely the ring gasket on the 8.625 casing head as it had been tightened recently. Mr. Shaw stated hew would schedule the lease operator to excavate the well head area and make necessary repairs to stop the leaking. No violations were observed during this inspection.
