0972265731 | 2016-11-03 | Production Wells | No Violations | The purpose for my inspection was to conduct a status check on well # 2-0821. A review of RBDMS indicates the Owner is current with reporting production. The lease road is stable and drivable. The well location is within an agricultural field and is stable. Correct and legible identification is posted at the well head. The 8.625 casing head is below grade. The 4.5 production casing is equipped with a casing gas powered pump jack. The pumping unit was operating during the inspection. I observed the stuffing box packing to be worn and leaking production fluid. Fresh production fluid was present on the well head assembly. Production fluid was pooling on the ground around the well head but not migrating away from location. A casing pressure gauge indicated 138 psi. A domestic supply connection was present. The tank battery consists of one (1) horizontal separator and one (1) 100 bbl production tank. An earthen dike that is stable and sufficient was present around the production tank. The separator was on a benched out slope immediately above the diked area. Minimum spacing requirements are met. The separator remained in good repair. No audible or visual leaks were detected. A current 7 day chart was in the meter house. Standing water that appeared fresh was present within the earthen dike. Evidence of a previous production tank overflow in the form of dried crude oil existed on the interior walls of the dike. No evidence existed that production fluid escaped outside of the earthen dike containment. An unopened bag of Peat Sorb was also present outside of the earthen dike. I tested the standing water within the containment area with both a low and high range Quantab. The sample exceeded the maximum range for the low range Quantab. The high range Quantab registered a value of 1121 ppm. The production tank remained in good repair with no detectable leaks. The load line valve was closed and plugged. The brine drain valve was closed and plugged. |

Mr. Wittick stated that the all repairs had been made and affected areas within the dike were cleaned up. I communicated to Mr. Wittick that I would plan to re-inspect the facility and make further contact if additional action was deemed necessary. |

Upon my re-inspection I found the well head and ground area adjacent to the well head cleaned of free production fluid. The pumping unit was not operating to determine if repair to the stuffing box packing was effective. The interior of the earthen dike was found to be free of free standing water. The walls of the earthen dike were found to be improved from my prior inspection that found them sufficient. The Owner also had fresh crushed gravel placed in the bottom of the containment area. At this time, no further action is necessary. |

Mr. Wittick stated that he did not personally operate this well, that it was subcontracted out. I informed Mr. Wittick that the stuffing box packing was in poor repair, causing production fluid to leak onto the well head and collect on the ground surface adjacent to the well head. Also, I brought attention to the free standing water within the earthen dike. Mr. Wittick acknowledged my concerns and stated he would schedule repair and clean up work as soon as possible. I extended a deadline of two weeks to have all corrective action completed, and that I would be contacted to confirm the work was completed. |
