0646983815 2017-06-14 Production Wells No Violations The purpose of my inspection was to document the posting of legible identification, repairs to the ground union inside the separator and improvements to the dike. At the time of my inspection, identification was not present at the well head and/or tank battery. I called and spoke with Dorfman Field Manager Paul Shaw on 6/19/2017 regarding the identification and dike improvements. Mr. Shaw stated he would schedule to have the identification posted and dike improved immediately. I will follow up with Mr. Shaw in a week to confirm the work was performed and schedule a follow-up inspection.
0857141264 2016-12-29 Production Wells No Violations The purpose of my inspection was to conduct a status check of well #2--0775 consisted of one (1) horizontal separator, one (1) 100 bbl production tank and one (1) concrete brine vault. Proper identification was not present on any of the production related equipment. The separator was located outside of the earthen dike. I observed production fluid to have leaked onto the floor of the separator from the motor valve unit. The meter house contained a current 7 day production chart. The production tank and vault were enclosed by an earthen dike that is worn on the East side and insufficient for containment. The location in proximity to a surface water stream creates a clear and present hazard. The interior of the dike contained freshwater that had become frozen. The production tank remained in good repair. No leaks were detected. The load line valve was closed and plugged. The brine drain valve was closed and plumbed to the vault. The hatch lid was closed. The brine vault above grade remained in good repair. A de minimis volume of production fluid was contained within the vault. I called and spoke with contract operator Jeremy Kohler concerning the missing identification, leaking separator and insufficient dike. Mr. Kohler stated he would have proper identification posted within a week along with making necessary repairs to the separator valve. Mr. Kohler also acknowledged the condition of the dike. He and I a
1216002415 2017-01-25 Production Wells No Violations The purpose of my inspection was to follow-up on the installation of an identification sign. Correct and legible identification has been posted on the production tank. The interior of the dike still contained water. No improvements to the dike have been made prior to this inspection. While on location I met contract lease operator Mark Kohler. We discussed the status of the dike and agreed that weather conditions have prevented improving the dike walls. I will continue to follow-up with Mr. Kohler as to when improvements will be made.

Correct Identification was present at the tank battery. The earthen dike wall height was increased to sufficiently contain a release. The dike was stable and free of fluid. No further action is necessary.
