0720175924 2017-04-03 Production Wells No Violations The purpose of my inspection was to witness the repairs made to the well head annular gas leak observed on my previous inspection. The well head was exposed and the annular side ports contained a plug in one side and a riser with a closed valve on the other side. Gas was observed venting from a pressure regulating valve. I called and spoke with Alliance Field Foreman Matt Postelthwaite who stated that the well was produced for oil and supplied domestic gas. Excess gas was periodically vented when excess pressure was reached. I requested that a charcoal filter be installed on the flow line. Mr. Postlethwaite stated he would schedule the filter installation. No violations were observed during inspection.
1486126565 2017-01-17 Production Wells No Violations The purpose of my inspection was to conduct a status check on well #2-0627. A review of RBDMS indicates the Owner last reported production in 2014. Lease access is off of a residential driveway that remains stable and in good repair. The well location is residential yard and is stable with well-established vegetation. Correct and legible identification is posted at the well head. The 8.625 casing head is below grade. The 4.5 production casing is equipped with a casing gas powered pump jack. Production valves were in the open position. Gas was observed leaking through water ponded on top of the casing head. Production fluid was observed to be leaking from the stuffing box and collecting on the ground. No production fluid was migrating off of location. The tank battery is shared with #2-1568. An earthen dike that is stable is present around the production equipment. The interior of the dike remains free of any fluids. Production equipment inside the dike for well #2-0627 consists of one (1) 210 bbl production tank.. Correct and legible identification was conspicuously placed on the tank body. The tank body remained in good repair with no detected leaks. The load line valve was closed and contained a plug. The drain line valve was closed and plugged. The hatch lid was closed. I contacted Justin Smith at Alliance Petroleum regarding the absent production and well head leaks. Mr. Smith stated that he would investigate the production issue and would be in contact with the field foreman to schedule the necessary repairs.
1497535979 2017-06-15 Production Wells No Violations The purpose of my inspection was to document the installation of a charcoal filter. I received a voice mail message from Alliance Field Supervisor Matt Postelthwaite stating that the filter had been installed down stream of the casing pressure regulator. At the time of my inspection the filter was installed and I could not detect venting gas. No other violations were observed. No further action is necessary at this time.
2015524942 2017-03-14 Production Wells No Violations The purpose of my inspection was to document the repair of the well head leak. The well head was cleaned of production fluid. I did not observe recent excavation of the ground adjacent to the well head to investigate my previously observed subgrade well head leak. I spoke with Alliance field superintendent Matt Postlethwait who stated the well head ring had been tightened and production fluid cleaned up. Mr. Postlethwait stated that at the time of the well head repair no gas was observed leaking around the well head. He continued that he would have their contract repair company excavate the well head area and investigate the leak as late as 3/22/2017. I will follow up with Mr. Postlethwait at the end of next week to determine the source and status of the repair.
