0057672009 2017-09-06 Plug / Plug Back No Violations The purpose of my inspection was to discuss attempt to remove the production/intermediate casing on well #2-0501. Northeast District Supervisor Steven Ochs and Mahoning County Inspector John Fleming were present for the meeting. Operator Bob Foltz attempted to remove the casing when the casing collar failed at 55 below surface. The decision was made to attempt to recover additional casing. Mr. Foltz planned to attempt to reconnect to the parted connection and utilize a casing ripper to separate the casing above the top of cement to achieve the recovery. I will schedule to be on location during future attempts.
0593775284 2017-09-13 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging activies on well #2-0501. Dave Miller was on location operating a casing ripper for Owner Bob Foltz in attempt to removed additional 5 casing. The rig weight indicator was indicating 20,000 pounds. The casing was beginning to pull free. The earthen pit remained holding fluid and maintained free board. No violations were observed during this inspection.
1304245343 2017-09-14 Plug / Plug Back No Violations The purpose of my inspection was to witness the attempt to remove the production/intermediate casing from well #2-0501. Operator Bob Foltz was able to screw back into the parted casing at 55 below surface. Mr. Foltz connected a pump to the casing to pump freshwater through openings left by a casing ripper. No circulation was established at surface. Mr. Foltz made repeated attempts to pull the casing by applying no more than 16,000 pounds of weight. The casing was coming free when the joint at 55 feet below surface failed. Mr. Foltz ceased plugging operations to seek a better option to remove the casing. I will schedule to be on location to witness further attempts to pull casing.
1443121323 2017-10-11 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging activates on well #2-0501. Owner Bob Foltz reported installing tubing in the well to a depth of 450 feet below surface to act as a tremie line. The static water level was measured at 150 feet below surface. R.W. Sidley delivered 5.0 yards of 9 sack grout. The grout was poured into the annular space of the tubing. Grout was brought to surface before flow was observed from the tremie line. Five joints of tubing were removed from the well and additional grout was poured into the annular space and brought to surface again. The remaining tubing was removed from the well and the remaining grout poured into the well bore and brought to surface. The grout held at surface. The location remained stable. The lined earthen pit was holding fluid and free board was maintained.
1506090379 2017-09-06 Plug / Plug Back No Violations I was on location to witness the plugging of the above well Foltz trying to rip 5.5 casing with a ripper. He is gone to try and get a new tool and try ripping tomorrow 9/7/2017.
1506092220 2017-09-07 Plug / Plug Back No Violations I was on location to witness the plugging of the above location. Foltz reworked the ripping tool and is trying to rip 5.5 casing. He is gone to contact Dave Miller a local operator for some advise.Shut down for the day.
1528812080 2018-06-12 Final Restoration No Violations The purpose of my inspection was to determine the status of final restoration on well #2-0501. At the time of my inspection, the working pit was backfilled and the location graded to local contour. Vegetation was well established. No signs of erosion or sedimentation were observed. All equipment and debris utilized in plug and abandonment was removed. The lease road was shared with well #2-1233. The location has passed the requirements for final restoration.
1554553521 2005-03-08 Production Wells Violations Noted The tank battery is shared with permits 0503 and 0500 and consists of 2-100 bbl storage tanks and noe dike. The well is idle and lacks identification. Only 5 bbls of oil has been produce since 1984.
1806823314 2017-10-10 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging activates on well #2-0501. Inspector John Fleming was on location to witness the removal of the production/intermediate casing and surface casing. Owner Bob Foltz acquired a fishing spear that facilitated the removal of the production/intermediate casing. A total of 354.15 of 5.1875 production/intermediate casing and 253.07 feet of 6.625 surface casing was removed from the well. Mr. Foltz reported that he was unable to schedule a delivery of 9 sack grout on 10/10/2017. Mr. Foltz planned to install tubing in the well to act as a tremie line and place the surface plug on 10/11/2017. I will schedule a representative of ODNR-DOGRM to be present to witness the surface plug.
2058028029 2015-11-18 Production Wells No Violations My inspection found the well to be equipped with a pumpjack powered by an electric motor. The well is constructed with 8 1/4 casing, 6 5/8 casing, and 5 3/16 casing. There is 2 tubing and rods in the well. The annulus is open to the atmosphere. The tank battery has been removed by the well owner. The last reported production for the well was 5 barrels of oil and it was reported in 1999. The well owner is currently working to plug berea wells on the property.
