0085532153 2018-06-12 Final Restoration No Violations The purpose of my inspection was to determine the status of final restoration on well #2-0464. At the time of my inspection, the working pit was backfilled and the location graded to local contour. The surface owner had the location planted in crop. No signs of erosion or sedimentation were observed. All equipment and debris utilized in plug and abandonment was removed. The access road was reclaimed. The location has passed the requirements for final restoration.
0608452333 2017-07-18 Plug / Plug Back No Violations The purpose of my inspection was to witness the setting of the bottom plug in well #2-0464. Mahoning County Inspector John Fleming was also present during operations. Operator Bob Foltz was unable to bail the well dry. Static water level was tagged at 160 feet below surface elevation. Total measured depth in the well was determined to be 843 feet below surface elevation. A tremie line of 2 inch tubing was placed in the hole to a depth of 819 feet below surface. R.W. Sidley delivered 1.5 yards of 9 sack grout. The grout was placed down the backside of the tremie line and displaced water out of the tremie line into a lined pit. Mr. Foltz periodically raised and lowered the tubing string to facilitate displacement. After the line stopped flowing the tubing was raised and lowered again prior to removal. The tubing string was removed and each joint pulled dry. I will schedule to be present during follow up activities to determine top of cement plug and witness attempts to pull casing.
1117671408 2017-07-27 Plug / Plug Back No Violations The purpose of my inspection was to witness plugging operations on well #2-0464. Operator Bob Foltz was on location and bailing crude oil from the well into the lined earthen pit. Per Mr. Foltz, 821 of tubing was removed from the well and total depth was tagged at 824. Fluid level was tagged at 100. All depths were measured from ground level. Mr. Foltz planned to bail the well dry on 7.27.2017, followed by a return visit on 7.28.2017 to assess recovery and plan to clean the well to the original total depth.
1325314714 2017-07-31 Plug / Plug Back No Violations The purpose of my inspection was to witness the placing of the bottom plug in well #2-0464. I witnessed the final bailer run that removed approximately 5 gallons of fluid from the well. Approximately 1.75 yards of 9 sack ground was poured into the open well bore from surface. The top of cement was tagged inside of the production casing. Owner Bob Foltz ran back into the well bore and removed two bailer volumes of grout in order to enable pulling the casing. The location remained stable. The earthen pit maintained two feet of free board. No violations were observed during this inspection.
1484102667 2017-07-26 Plug / Plug Back No Violations The purpose of my inspection was to conduct a status check on well #2-0464. The Foltz service rig was set up on the well. The location remained stable. A pit to service plugging operations was dug and lined with an impervious liner. No spills or leaks were observed. While on location I called and spoke with owner Bob Foltz concerning the status of plugging operations. Mr. Foltz stated that he planned to be on location on 7.27.2017 to begin attempting to bail the well dry in order to set the bottom plug. I will schedule to be on location to witness bailing operations.
1579686216 2017-08-01 Plug / Plug Back No Violations The purpose of my inspection was to witness the attempt to pull casing from well #2-0464. Operator Bob Foltz measured the top of cement at 563. The 5 casing seat was set at 592 Mr. Foltz was able to recover 592 of 5 casing from the well. The last joint and a half contained grout. The location remained stable. The lined earthen pit maintained adequate free board. Mr. Foltz planned to return on 08.02.2017 to attempt to remove 7 casing from the well and to place the top plug of 9 sack grout. No violations were observed during this inspection.
1681768901 2015-11-18 Production Wells No Violations My inspection found the well to be equipped with 8 1/4 casing, 6 5/8 casing, and 5 3/16 casing. There is 2 tubing and rods in the well. The pump jack has been removed from the well. The annulus is open to the atmosphere. The tank battery has been removed by the well owner. There is no known production ever recorded for the well. The well owner is currently working to plug other berea wells on the property.
2109940137 2017-08-02 Plug / Plug Back No Violations The purpose of my inspection was to witness the attempt to pull 7 casing and set the top plug of 9 sack grout. Operator Bob Foltz was able to recover 260 of 7 casing. A measuring line was placed in the hole and a bridge was encountered at 417 Mr. Foltz was unsuccessful in attempting to clear the bridge with tubing. Fluid level was tagged at 150. Five yards of 9 sack grout was ordered and placed into the well bore utilizing a tubing string for a tremie line. Approximately 4 yards of grout was placed into the well when grout was observed at surface. The tubing string was removed and the well bore topped off with grout. Northeast Regional District Supervisor Stephen Ochs was present during grout plug operations. No violations were observed during this inspection.
