The side gate valve is plumbed into a 4.00 lead line that runs into the main line. The well is properly identified with a clear, legibly marked well identification sign. The lease road is in stable condition and drivable. No violations found.
0283657368 2018-05-09 Storage Well No Violations The purpose of my inspection was to conduct a status check on well #2-0456. Access to the well remained stable and drivable. Correct identification was present at the well head. The surface casing well head was above grade and closed to atmosphere. The production casing was equipped with a flow tee that contained a capped swab valve on the top and a wing valve on the side. The wing valve connected to the main line. No leaks were witnessed. No violations were observed during this inspection.

2104386673 2017-03-06 Storage Well No Violations The purpose of my inspection was to conduct a status check on well # 2-0456. Lease access was over an agricultural road that was stable and drivable. The well was located within an agricultural field stabilized with well-established vegetation. Correct and legible identification was present at the well. Well head annular side ports contained a plug in one side and a closed and plugged valve in the other side. The top of the well head was equipped with a production tee. The top and side of the tee contained gate valves. The top gate valve was capped. A 4 line was plumbed out of the side gate valve to the main line. Test port valves were closed and plugged. No audible or visual leaks were detected. No violations were detected during this inspection.
