0357359279 2018-06-12 Plug / Plug Back No Violations The purpose of my inspection was to document the status of plugging activity on well #2-0207. Owner Bob Foltz was rigged up on the well and disassembling the well to pull the rods and tubing from the well. The location was stable. I left location to conduct other status inspections and returned later in the day and found the lift equipment removed from the well. The well was open and I did not observe any flow from the well. I will schedule to be present to witness plugging activities.
0601441654 2019-01-15 Final Restoration No Violations The purpose of my inspection was to document the final restoration status of well #2-0207. The well location was at the boundary of an agricultural field. At the time of my inspection the location was partially covered in snow. The pit was backfilled. No equipment remained. No buried flow lines existed. No debris was present. No production equipment was on location. The location was stabilized with vegetation. I did not witness any signs of erosion. No access road was present. The location meets the requirements for final restoration.
0752960529 2018-06-14 Plug / Plug Back No Violations The purpose of my inspection was to witness plug and abandonment activities on well #2-0207. Owner Bob Foltz reported that he was unable to bail the production zone dry. Initial fluid level was measured at 410 below surface and was A tremie line of 2 tubing was inserted to a depth of 715 below surface. Total depth was determined to be 722 below surface. R.W. delivered and poured 1.25 yards of 9 sack grout into the well bore. As grout was poured into the well bore, the tremie line circulated fluid out of the well bore into the lined pit. After setting the bottom plug, the tremie line was removed and the rig crew set up to attempt to pull casing. Approximately 67,000 pounds was pulled against the surface casing to facilitate removal. A total of 450 of 6.625 OD casing was removed from the well. The tremie line was reinstalled into the well to a depth of 404. After installing the tremie line, the static water level in the well was at grade. R.W. Sidley delivered and poured 4.25 yards of 9 sack grout into the well bore. The tremie line circulated fluid into the lined pit. The grout was brought to surface at which time the tremie line was removed. After removing the tremie line, the well bore was filled back to surface. The plug held at surface. The earthen pit was holding fluid with freeboard. I will schedule to witness the plug height prior to cut off and back fill. No violations were observed during this inspection.

A pumping rod extended from the top of the stuffing box. No pumping unit was present on the well head. No audible or visual leaks were identified. I did not observe any production equipment. I will contact orphan well inspector Heidi Scott to determine if the well was operated or assigned to the owner of other wells on the same surface property.
