-in at the time of inspection due to repairing a heat treater gasket. The operator indicated a ten ounce release of condensate which was on containment and remediated. Personnel were currently completing repairs while we were on site. No violations were noted at the time of inspection.

0590050054 2017-07-26 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with an open-flow assembly. I spoke to Chesapeake personnel on site, who informed me that flow-back operations were completed on 07/25/2017 and the well was in production. Jersey barriers were installed around the wellhead. Proper identification was posted at the wellhead. Identification was posted at the tank battery which was surrounded by a dike and fifteen 400 bbl production tanks, five labeled as containing produced water, and fifteen labelled as containing condensate.. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

15- 400 bbl production tanks, five labelled as containing fresh water, and fifteen labelled as containing condensate 8 producing wells 8 sand separators 1 horizontal separator 1 low-pressure vertical separator 8 gas processing units 2 combustor units 3 oil stabilization units 3 compressors Jersey barriers are installed around the production equipment. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The lease road was stable in in good repair.

0723368089 2018-06-04 Production Wells No Violations I was on site in response to complaint #5671 regarding vibrations at the complainants house who lives near the pad. I received a call from the complainants residence at approximately 9:00 AM on 06/04/2018 stating vibrations were felt at the residence. I arrived on site at approximately 9:30 AM. No equipment malfunctions or anomalies were observed on site. I stood near the combustor for several minutes and did not hear or detect any anomalies at the time of inspection. No violations were observed on the pad.

The surface-intermediate annular gauge displayed a reading of zero psi and intermediate-production annular gauge displayed 220 psi. I performed a walk-around inspection of the site and no violations were observed at the time of inspection.

1073501267 2017-09-21 Production Wells No Violations I was on site in response to complaint #5671 to listen for any abnormalities on site after receiving a call from the complainant at 8:50 AM stating they were experiencing vibrations at the house. The noise coming from the combustor was more noticeable on the pad than I have witnessed during previous inspections. I spoke to Mark Deering of Chesapeake on site, who informed me that the production team routinely adjusts the louvers to the combustors when needed.
1190703953 2017-08-20 Production Wells No Violations I was on site in response to complaint #5671 in which the complainant, who lives near the pad, is reporting incidents of vibrations at his home. No equipment malfunctions or noise/vibration anomalies were observed at the time of inspection. Following the inspection, Mike Mann of the Division of Mines and I met with the complainant to review seismic data gathered from equipment the equipment deployed by the DOGRM seismic section on 08/15/2017.
1288214153 2017-08-14 Production Wells No Violations I was on site in with Stephen Ochs in response to complaint #5671 in which the complainant, Mr. Kiefer, who lives approximately 0.5 miles south of the pad was reporting vibrations at his house. No noise/vibration anomalies, or equipment issues were observed at the time of inspection.
1442258039 2015-09-11 Casing String Witnessed No Violations I was on location for the cementing of the conductor casing. The permit number and the emergency information were posted at the entry of the lease road. The pad was surrounded by a chain link fence with a main gate. The lease road was stable and drainage controls were in place. Vegetation has been established on the location to prevent soil erosion. Express Energy Services drilled a 26 diameter hole to 120. Express installed 120 of 94lb, 20 OD casing. The driller had no issues to report while drilling the hole. Express was the cementing contractor on location. Express rigged up and broke circulation. The cement was mudscaled at 15.6 pounds per gallon (ppg). Express pumped 48.34 bbls of cement slurry at 15.6 ppg. The cement was displaced with 39 bbls of water. Cement returns to surface were observed. An estimated 12 bbls of cement returned to surface.
