Express Energy was the cementing contractor. They pumped 230 sacks (48.33 bbl..) of class A cement 15.6 lb./gal. The cement was displaced with 39.07 barrels of fresh water. There was 17 barrels of cement returned to surface.
0686251093 2015-04-11 Drill / Deepen / Reopen No Violations Nomac Rig 70 drilled a 12.25 hole to a depth of 2506 and ran 2503 of 9.625 OD 40 lb/ft of intermediate casing with 20 centralizers. O-Tex was the cement contractor. O-Tex pumped 10 barrels of fresh water followed, by 35 barrels of Bentonite Gel followed, by 10 barrels of fresh water. They pumped 900 sacks of Premium H 15.8 ppg cement (193.95 bbls). The plug was displaced with 182.8 barrels of water. Plug landed and pressure was increaced to 1500 psi., floats held when pressure was realeased. There was 40 barrels of 14.6 ppg barrels of cement returned to surface.

The surface-intermediate annular gauge displayed a reading of six psi and the intermediate-production annular gauge displayed 150 psi. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
1429632117 2015-04-08 Drill / Deepen / Reopen No Violations I met with Kevin Chaney Drilling Consulant for Chesapeake Exploration on Nomac Rig 70. We went over the drilling permit and condition on this well. This is a multi well drilling pad. At present the rig is 90% rigged up. They will notify me when they start drilling. The surface hole ( 17.5 ) and the intermediate hole ( 9.625 ) will be logged.
1616196641 2018-06-19 Production Wells No Violations The well was equipped with an open-flow assembly. The surface-intermediate annular gauge displayed a reading of zero psi, and the intermediate-production annular pressure gauge displayed readings of 300 psi. No changes have been made to the pad equipment since my last inspection. I performed a walk-around inspection of the pad and no violations were observed at the time of inspection.
1722882837 2017-11-13 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped an open flow Christmas tree assembly. Jersey barriers were installed around the wellhead and the well was identified with a legibly-marked well identification sign. The surface--pressure vertical separator. The tank battery is surrounded by a dike and the tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Production equipment on the well pad consisted of:

8- 400 bbl production tanks, three labelled as containing fresh water, and five labelled as containing condensate, with a low pressure vertical separator located inside of the dike. six producing wells six sand separators six gas processing units one heat treater one combustor unit two compressor stations one oil stabilization unit

Jersey barriers are installed around the production equipment.  The access road and man doors were locked.  Drainage and erosion controls at the pad are in place and functioning.  A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure.  The pad has a crushed limestone surface, and is in good repair.  The lease road was stable in in good repair.
2003748959 2017-06-27 Drill / Deepen / Reopen No Violations I was on site during hydraulic fracturing completions of the #2H (PN#22665), #8H (PN# 22668, and #9H ((PN# 22667) wells after the casing on the #2H and #7H wells were repaired in May of 2017. Casing issues were encountered during completions operations on the #6H well in April of 2016, and the casing was repaired in April 2016. Hydraulic fracturing on the #6H well was completed in January or 2017. I conducted a status check of the well and site while on location. No issues were noted at the well or pad at the time of inspection.

O-Tex was the cement contractor. O-Tex pumped 10 barrels of fresh water followed, by 35 barrels of Bentonite Gel followed, by 10 barrels of fresh water. They pumped 790 sacks of Premiun H 15.6 ppg cement (168.84 bbls). The plug was displaced with 127 barrels of water. Plug landed and pressure was increaced to 1000 psi., floats held when pressure was realeased. There was 37 barrels of 14.9 ppg barrels of cement returned to surface.
