0295171541 2018-05-24 Drill / Deepen / Reopen No Violations I was on site during preparations of the well for stimulation operations, which were anticipated to commence on 05/25/2018. I reviewed the bond logs of the 5.5 cement job, and I interpreted the top of cement to be at approximately 3,090. Kick-off point was reported to be at 7,046. I performed a walk-around inspection of the site and no violations were observed at the time of inspection.

0400785431 2018-11-26 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. Three producing wells (22745, 22744, 22642) are located on the pad. This well is open flow with a wellhead Christmas tree assembly. The surface-intermediate annular pressure gauge displayed a reading of zero psi, and the intermediate-production gauge displayed 35 psi. Jersey barriers were installed around the wellhead. The tank battery consisted of eight 400 BBL storage tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipaters. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:

eight storage tanks; five labelled as containing condensate, and three as containing produced water one low-pressure vertical separator three sand separators three gas processing units one combuster unit one oil stabilization unit Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface and was in good repair. The lease road is stable and drivable. Preliminary restoration passed.

I was on site while equipment was being moved on site for well stimulation. The wellhead was equipped with a 15k frac valve. I spoke to the Chesapeake representative on site and reviewed the permit and notifications. Completion operations are expected to commence on 05/24/2018. No violations were observed at the time of inspection.

1845990006 2018-04-07 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the 13-3/8 Surface Casing. Patterson Rig 580 drilled to a total depth535 and ran 520 of 54.5 lb/ft of 13-3/8 casing. O-Tex Cementing tested there lines to 2500 psi and pumped 30 barrels of gel followed by 10 bbls of flush mix system. They pumped 110.5 bbls (580 sacks of class H cement 16.6 ppg 1.97 yld) >the plug was dropped and displaced with 73.6 bbls of fresh water. Plug landed with and held 500 psi over.
