0377885487 2016-03-01 Production Wells No Violations I conducted a status check of this well. Identification signs were posted at the access road entrance, the well, and the storage tanks. The access road gate was in good repair, but was unlocked at the time of inspection. The #3H well is in production. The well is equipped with a plunger lift assembly. The production tanks are surrounded by a steel dike. The pad is surrounded by a fence and equipped with locks on the man door. The man door was open at the time of inspection, but Chesapeake personnel were on site. The equipment on the pad consists of the following:

9 - 400 bbl production tanks, three labelled as containing produced water, six labelled as containing condensate. 1- Low-pressure vertical separator located inside of the dike of the tank battery. 7 Producing wells 7 Sand separators 7 Gas Processing Units 5 Line Heaters 1 Heat Treater 1 Vapor Recovery Unit 2 Bullet Tanks

0394709861 2014-09-02 Drill / Deepen / Reopen No Violations I was on location to check the status on Nomac #73. The rig was preparing to walk from the Kennedy #2H. They plan to spud the 17.5 surface hole tomorrow morning. The location is in good condition and everything is on containment. No violations.
0991626951 2015-10-08 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is open flow with a wellhead Christmas tree assembly. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of nine, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

1664123266 2014-09-05 Drill / Deepen / Reopen No Violations Nomac Rig 73 drilled a 12.25 hole to a depth of 2507.45 and ran 2505.2 9 5/8 J-55 40# intermediate casing with 20 centralizers. O-Tex pumped 35 barrels of bentonite gel spacer followed by 10 barrels of fresh water. They pumped 194 barrels of 15.8 cement 3% Gypsum,2% CaCl2, 0.8% MC-500 (BWOW), 0.5% C-45, 0.4% C-41P. Displaced with 183 barrels of fresh water. 37 barrels of cement returned to surface.
1870130289 2014-10-12 Drill / Deepen / Reopen No Violations I performed a status check on the above location Nomac 73 has TD the well and are rigging up the casing crew. They well be cementing sometime on 10/13/2014. They will notify me before cementing. Location is clean Drilling Permits and Conditions are posted. This is a multi well pad.
1908887845 2014-09-03 Drill / Deepen / Reopen No Violations Nomac Rig 73 drilled a 17.5 hole to a depth of 856.22 and ran 853.1 of 13 3/8 J-55 54.5# surface casing with 7 centralizers. O-Tex pumped 35 barrels of Gel spacer followed by 10 barrels of fresh water followed by 164.5 barrels of 15.6 class A cement. They displaced with 124 barrels of fresh water. Bumped plug and pressured to 1094 psi held for 5 minutes 1/2 barrel returned to tub. There was 44 barrels of cement returned to surface.
1954374505 2014-07-23 Casing String Witnessed No Violations I was on location to witness the cementing of the 20 conductor casing. The joints are butt welded and the casing is set open-ended on the bottom of a 26 hole that is 120 in depth. Holes have been cut in the casing 5-6 off bottom to allow fluid circulation. O-Tex Pumping cemented the casing with 47 barrels of 15.6lb/gal, Class H slurry. 20 barrels of cement slurry circulated to the surface. See cement report for more details on this job. No violations.
