0039098942 2014-07-19 Casing String Witnessed No Violations I witnessed the cementing of the conductor casing. TopHole Drilling drilled a 26 diameter hole to 120. TopHole installed 120 of 20 OD casing. During drilling, water was encountered at an approximate depth of 40. Allied Oil & Gas Services was the cementing contractor. The cement was mudscaled, and Allied pumped 51.9 bbls of cement slurry at 15.6 pounds per gallon. The cement was displaced with 39 bbls of water. 15 bbls of cement returned to surface. Displacement was completed at 1:30am.
1062769356 2014-07-22 Drill / Deepen / Reopen No Violations I was on location to check on Nomac #72 moving onto location from the Forcier #10H. They are currently moving in small loads (mud pits and pumps). The rig mats are down and covered with the containment layer. Estimated spud is 7/25/14. No violations.
1365586060 2014-07-26 Casing String Witnessed No Violations I was on location to witness the 13.375 surface casing cement job. Nomac #72 drilled a 17.5 hole to a depth of 710 and Wyoming Casing landed the pipe at a depth of 700. I spoke with the driller and he indicated that they had lost 1000 barrels of municipal water at the bottom of the conductor casing. I made sure that Schlumberger was going to run plenty of Cello-Flake during cementing operations. Schlumberger cemented the casing with 134 barrels of Class A cement slurry weighing 15.6lbs/gal. 30 barrels of cement circulated to surface. See cement report for more details on this job. 1417548812 2014-07-28 Casing String Witnessed No Violations I was on location to witness the 9.625 intermediate casing cement job. Nomac #72 drilled a 12.25 hole to a depth of 2343 and Wyoming Casing landed the pipe at a depth of 2332. Schlumberger cemented the casing with 181 barrels of Class A cement slurry weighing 15.6lbs/gal. 30 barrels of cement circulated to surface. See cement report for more details on this job. 1768868012 2015-12-03 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.
