0540815811 2015-12-03 Preliminary Restoration No Violations I was on location to conduct a preliminary restoration inspection. The well is equipped with a plunger lift unit. Jersey barriers were installed around the wellhead. The well is properly identified with a clear, legibly-marked well identification sign. The tank battery consisted of six, 400 BBL production tanks and was diked. The tanks are located within a spray-lined and sealed secondary containment. There are e-stops outside the barriers, and the tanks are grounded, and have static dissipators. There is a truck grounding area for trucks collecting condensate and/or produced water. Lightning protection and a windsock are installed on top of the production tanks. Other production equipment on the well pad consisted of:



Jersey barriers are installed around the production equipment. The entire well pad is enclosed with a six-foot high, chain link fence with three-strand barbed wire and the man doors were locked. Along the interior of the chain link fence there is an earthen berm and it was stable. The soil stockpile is well-stabilized with vegetation. Drainage and erosion controls at the pad are in place and functioning. A closed-loop, above-ground drilling pit system was used, so there are no earthen pits requiring closure. The pad has a crushed limestone surface, and is in good repair. The lease road is stable and drivable. Preliminary restoration passed.

0614396070 2014-08-13 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #72. They drilled the 8.75 section to the mud-up-point (MUP) of 6,700 and are currently drilling the 8.5 lateral hole at a measured depth of 13,794. They reach total depth and cement 5.5 production casing on Friday. The location is in good condition and all drilling equipment is on containment. No violations.
0637480186 2014-08-05 Casing String Witnessed No Violations I was on location to witness the cementing of the 13.375 surface casing. Nomac #72 drilled a 17.5 hole to a depth of 713 and Wyoming Casing set the pipe at a depth of 703. Schlumberger cemented the casing with 630 sacks of Class A cement weighing 15.6lbs/gal on the pressurized scales. 22 barrels of cement slurry circulated to surface. 0859601933 2014-08-07 Casing String Witnessed No Violations I was on location to witness the cementing of the 9.625 intermediate casing. Nomac #72 drilled a 12.25 hole to a depth of 2,512 and Wyoming Casing set the pipe at a depth of 2,502. Schlumberger cemented the casing with 885 sacks of Class A cement weighing 15.8lbs/gal on the pressurized scales. 23 barrels of cement slurry circulated to surface. 1406578277 2014-07-19 Casing String Witnessed No Violations I witnessed the cementing of the conductor casing. TopHole Drilling drilled a 26 diameter hole to 120. TopHole installed 111 of 20 OD casing. During drilling, water was encountered at an approximate depth of 40. Allied Oil & Gas Services was the cementing contractor. The cement was mudscaled, and Allied pumped 51.9 bbls of cement slurry at 15.6 pounds per gallon. The cement was displaced with 39 bbls of water. 17 bbls of cement returned to surface. Displacement was completed at 12:30am.
2074920715 2014-08-18 Drill / Deepen / Reopen No Violations I was on location to check the status of Nomac #72. They are currently rigging down to move the rig to the Jamar Pad in Harrison County. I met with the company men and picked up my paperwork from the 5.5 production casing cement job. No violations.
